微血管减压术治疗脑神经血管压迫综合征43例临床分析

目的探讨乙状窦后入路微血管减压术(MVD)治疗脑神经血管压迫综合征的临床疗效。方法回顾性分析我科自2009年6月至2011年6月来应用MVD治疗脑神经血管压迫综合征43例(面肌痉挛21例,三叉神经痛20例,舌咽神经痛2例)的临床资料,对其治疗疗效进行分析。结果20例三叉神经痛及2例舌咽神经痛患者术后疼痛立即消失;21例面肌痉挛患者中19例术后痉挛立即消失,2例术后3d痉挛消失。所有患者未见听力下降、面瘫、眩晕、血肿及脑脊液漏等并发症。术后随访2～24个月,43例患者均无复发。结论MVD治疗三叉神经痛、面肌痉挛及舌咽神经痛是安全有效的,创伤小,并发症少,复发率低,是目前治疗脑神经血管压迫综合征的最佳方法。

Clinical analysis on the therapeutic effect of microvascular decompression in the treatment of 43 neurovascular compression syndromes

Objective To analyze the therapeutic effect of microvascular decompression on neurovascular compression syndromes. Methods From June 2009 to June 2011, 43 patients of neurovascular compression syndromes treated by microvascular decompression were analyzed retrospectively, including 21 patients of facial spasm, 20 patients of trigeminal neuralgia, 2 patients of glossopharyngeal neuralgia. Results Pain was immediately abolished in the 20 patients of trigeminal neuralgia and the 2 patients of glossopharyngeal neuralgia. Spasm was immediately abolished in 19 of the 21 patients of facial spasm, which was abolished 3 days post-operation in the remaining two patients of facial spasm. There were no complications in all the patients, such as hearing low, facial palsy, dizzy, leakage of cerebrospinal fluid. No recurrence was found during the follow-up of 2～24 months. Conclusion Microvascular decompression is effective and safe for treating neurovascular compression syndromes, with mini trauma and low recurrence.
