三维适形与三维调强放射治疗乳腺癌保乳术后疗效对比分析

摘    要:目的对比分析三维适形与三维调强放射治疗乳腺癌保乳术后的临床疗效。方法选取2010年1月至2013年8月在新疆医科大学附属肿瘤医院接受保乳手术治疗的乳腺癌患者90例作为研究对象,采用随机数字表法将患者随机分为三维调强组和三维适形组,各45例,三维调强组采用三维调强法进行放疗,三维适形组采用常规三维适形法进行放疗,比较两组临床效果和皮损严重程度。结果三维强调组患者的皮损发生情况较三维适形组轻(P<0.05);三维调强组靶区照射剂量的D95显著高于三维适形组(4540.5±38.2)Gy vs(4416.0±70.6)Gy,P<0.05],V105%、V110%均显著低于三维适形组(18.3±7.9)Gy vs(27.8±8.5)Gy,(2.4±0.7)Gy vs(3.8±1.5)Gy,P<0.05]。结论三维调强法临床效果优于三维适形法,值得临床推广应用。

Abstract:Objective To compare the clinical effect of intensity-modulated radiotherapy (1MRT) and three-dimensional conformal radiotherapy(3-DCRT) on patients with breast cancer after breast conserving surgery. Methods A total of 90 cases of patients with breast cancer treated in Xinjiang Medical University Affiliated Tumor Hospitalfrom Jan. 2010 to Aug. 2010 were Selected as the research objects, which were randomly divided into two groups with random number table method ,45 cases in each group, with one group given IMRT and one group given 3-DCRT. The clinical effect and severity of skin damage of the two groups were compared. Results The skin damage in IMRT group was less severe than 3-DCRT group. D95 exposure dose of IMRT group was significantly higher than 3-DCRT group (4540.5 ± 38.2 ) Gy vs (4416.0 ± 70.6 ) Gy,P 〈 0.05 ], V105%, V110% were lower than 3-DCRT group (18.3 ± 7.9 ) Gy vs (27.8 ± 8.5 ) Gy, (2.4 ± 0.7 ) Gy vs ( 3.8 ± 1.5 ) Gy, P 〈 0.05 ]. Conclusion Clinical effect of IMRT is better than 3-DCRT, thus is worth of clinical application.
