经阴道超声对多囊卵巢综合征诊断价值的研究

摘    要:目的观察B超测量值对多囊卵巢综合征(PCOS)的诊断价值。方法对26例确诊为PCOS合并胰岛素抵抗患者、31例单纯PCOS患者及30例正常育龄妇女进行经阴道B超测量卵巢,共5个参数,分别为卵巢总体积(OV)、卵泡数(FN)、卵巢总面积(TA)、间质面积(SA)、间质面积和总面积比值(SA/TA)。结果OV、TA、SA/TA在PCOS合并胰岛素抵抗组、单纯PCOS组及正常对照组之间有显著性差异,FN在单纯PCOS组与正常对照组之间、PCOS合并胰岛素抵抗组与正常对照组之间有显著性差异,SA在正常对照组与PCOS合并胰岛素抵抗之间、单纯PCOS组与PCOS合并胰岛素抵抗组之间有显著性差异。结论在PCOS诊断中,经阴道B超测量FN、OV、TA、SA/TA可考虑作为PCOS的常规诊断指标。OV、TA、SA/TA的增大,尤其是SA的增大可提示诊断PCOS合并胰岛素抵抗。

Abstract:Objective To investigate the value of ovarian parameters changes by transvaginal B-ultrasound in diagnosis of polycystic ovary syndrome(PCOS).Methods Ovarian volume(OV),number of the small follicles(FN),stromal area(SA),total area(TA),and stromal area/total area ratio(S/T) of 26 patients of PCOS combined with insulin resistance and 31 PCOS patients and 30 volunteer women controls were measured by transvaginal B-ultrasound.Results OV,TA,SA/TA were significantly different among the PCOS combined with insulin resistance group,the PCOS group and the control group.FN was significantly different between the PCOS combined with insulin resistance group and the control group,between the PCOS group and the control group.SA was significantly different between the control group and the PCOS combined with insulin resistance group,between the PCOS group and the PCOS combined with insulin resistance group.Conclusion FN,OV,TA,SA/TA ratio might be considered as a B-ultrasound diagnostic parameter in PCOS.The more increasing of OV,TA,SA/TA,especially the latter,implied the more possibility of PCOS combined with insulin resistance.
