5576例高血压患者临床联合使用丹红注射液的用药分析

目的：基于临床数据分析高血压患者联合使用丹红注射液的情况,为后续开展丹红注射液与降压药物的药物相互作用实验研究提供数据支持。方法：采用回顾性研究方法,选取南京中医药大学附属医院医院信息系统数据库中2013年1月-2015年12月使用丹红注射液的5576例高血压患者的基本资料和医嘱信息,运用频数统计和关联规则方法挖掘高血压患者的并发症关联情况,及丹红注射液与降压药物的联合用药规律,并针对联用频数较多的药物组合进行药物相互作用文献分析。结果：高血压并发症关联统计结果显示：置信度最高的并发症为心肌病、心律失常、脑梗塞和冠心病;联合用药关联统计结果显示：丹红注射液常与抗血小板药、调血脂药、脑血管疾病用药和各类降压药物等联用。其中美托洛尔和氨氯地平在与丹红注射液单项合并用药中联用频次排序前十,且均有开展药物相互作用深入研究的价值。结论：高血压患者使用丹红注射液的联合用药复杂,但药物相互作用研究薄弱,有必要后续开展与降压药物的相互作用实验研究,为临床用药风险预警和安全性评价提供证据。

Analysis of concomitant use of Danhong injection in 5576 cases of hypertension patients in clinic practice

OBJECTIVE To provide data support for the subsequent experimental study of the drug interaction between Danhong injection and antihypertensive drugs based on the clinical data analysis of the combined use of Danhong injection in patients with hypertension.METHODS A retrospective study was conducted based on 5576 cases of patients with hypertension in HIS database of the affiliated hospital of Nanjing University of Chinese Medicine from January 2013 to December 2015. Frequency analysis and association rule analysis were to explore the correlation of complications in patients with hypertension, and the drug combinations of Danhong injection and anti-hypertension drugs. Reference analysis for the typical drug combinations with more frequecy was carried out for further investigations.RESULTS The statistical results showed that the highest confidence of the complications were cardiomyopathy, arrhythmia, cerebral infarction and coronary heart disease.And Danhong injection was often used in combination with antiplatelet agents, lipid-regulating drugs, cerebrovascular medication and various antihypertensive drugs. Among them, metoprolol and amlodipine ranked the top ten, and both of which were worthy of further investigation CONCLUSION The concomitant medication in patients with hypertension using Danhong injection is complex with a lack of related researches. It is recommended to carry out subsequent experimental study on the interaction with antihypertensive drugs to provide evidence for clinical drug risk warning and safety evaluation.
