超声宫腔水造影在宫腔病变中的应用评价

摘    要:目的探讨超声宫腔水造影在诊断常见宫腔病变中的应用及评价。方法回顾采用经阴道二维联合三维超声、宫腔水造影检查并行宫腔镜或手术诊刮取材行病理检查的469例病人资料,分别将经阴道二维联合三维超声和宫腔水造影术对宫腔占位病变的诊断结果与手术结果对照,比较分析2种不同超声方法对宫腔病变诊断试验评价指标(灵敏度、特异度、符合率)。结果以手术为标准,经阴道二维联合三维超声诊断宫腔息肉、黏膜下肌瘤、宫腔粘连的灵敏度、特异度和符合率均显著低于宫腔水造影(P < 0.01)。结论宫腔水造影对宫腔病变的诊断较常规超声具有优势,可以作为临床不孕症病人寻找不孕原因的一种筛查手段。

Application value of Saline infusion sonohysterograph in the diagnosis of uterine cavity lesions

Abstract:ObjectiveTo evaluate the application value of saline infusion sonohysterograph(SIS) in the diagnosis of common uterine lesions.MethodsThe clinical data of 469 patients detected by transvaginal two-dimen sional (2D) combined with three-dimen sional (3D) ultrasound, SIS and pathological examination were retrospectively analyzed.The results of transvaginal 2D combined with 3D ultrasound and SIS in the diagnosis of uterine cavity lesions were compared with the results of operation.The sensitivity, specificity and coincidence rate between two ultrasonic methods in the diagnosis of uterine cavity lesions were compared.ResultsWith hysteroscopy as the standard, the sensitivity, specificity and coincidence rate of 2D combined with 3D ultrasound in the diagnosis of uterine cavity polyps, submucosal leiomyoma and intrauterine adhension were significantly lower than those of SIS(P < 0.01).ConclusionThe SIS has some advantages over conventional ultrasound in the diagnosis of intrauterine lesions, and can be used as a screening method for clinical infertility patients to find the cause of infertility.
