肾癌145例治疗分析

目的 探讨肾细胞癌的诊治方法，进一步提高其疗效。方法 对145例肾癌手术治疗的临床资料进行回顾性分析。结果 诊断以B超，CT为主。行肾癌根治性切除125例，其中20例因肾脏肿瘤较大或有肺转移先行肾动脉栓塞术。5例同时行下腔静脉切开取栓术，手术探查3例。病理结果：透明细胞癌104例（71.7%)，颗粒细胞癌18例（12.4%），乳头状肾癌5例，混合型癌18例（12.4%)。135例（93.9%)获得随访。3、5、10年生存率分别为70.2%(51/70)、56.2%(18/32),27.2%(3/11)。结论 B超和CT检查是目前诊断肾癌的重要手段，根治性肾癌切除术仍是最有效的治疗方法。

Clinical Analysis of 145 Cases of Renal Cell Carcinoma

Objective To probe into the strategies for the diagnosis and treatment of renal cell carcinoma in order to further raise its curative effect.Methods Betrospective analysis was made of the information about the surgical operations for 145 cases of renal cell carcinoma.Results Diagnosis was made chiefly through ultrasonography and CT scanning.125 cases underwent radical nephrectomy of these,20 cases underwent renal arterial embolism first,because of their comparatively larger sized tumours or because of metastasis to the lungs,while 5 cases simul taneously underwent cave incision to remove the embolism.And surgical exploration was done for 3 cases.Pathology indicated:104 cases of clear cell carcinoma (71.7%),Grannular cell carcinoma in 18 cases ( 12.4% ),5 cases of papilliform renal carcinoma,18 cases of the mixed type renal carcinoma( 12.4% ).Follow ups were achieved in 135 cases (93.95),with the 3 year and 10 year survival rates being 70.2% (51/70),56.2%(18/32)and 27.2%(3/11) respectively.Conclusion Ultrasonography and CT scanning remain important diagnostic tools currently available in the diagnoses of renal cell carcinoma;while radical renal nephrectomy continues to be the most effective approach to deal with this malignancy.
