消疽汤口服联合活血通络汤熏洗治疗气虚血瘀型糖尿病足临床研究

目的:观察消疽汤口服联合活血通络汤熏洗治疗气虚血瘀型糖尿病足的临床疗效。方法:128例气虚血瘀型糖尿病足患者随机分为对照组和观察组,每组各64例。所有患者均接受糖尿病常规治疗,对照组在常规治疗基础上给予阿司匹林+胰岛素+庆大霉素+山莨菪碱,观察组在常规治疗基础上给予消疽汤口服联合活血通络汤熏洗。比较两组患者治疗前后临床症状、神经传导速度、踝肱指数及不良反应,并评价两组患者临床疗效。结果:治疗后,观察组中医证候积分为(8.25±2.33)分,明显低于对照组的(13.58±2.24)分,差异有统计学意义(P0.05)。观察组治疗后的正中神经、腓总神经运动神经传导速度分别为(46.57±3.84)m·s~(-1)、(43.02±2.71)m·s~(-1),均明显高于对照组的(42.58±2.78)m·s~(-1)、(39.58±4.12)m·s~(-1),差异均有统计学意义(P0.05);观察组治疗后的正中神经、腓总神经感觉神经传导速度分别为(43.02±2.84)m·s~(-1)、(40.82±3.42)m·s~(-1),均明显高于对照组的(39.88±2.21)m·s~(-1)、(35.58±3.38)m·s~(-1),差异均有统计学意义(P0.05)。观察组治疗后踝肱指数为(0.98±0.29),明显高于对照组的(0.79±0.21),差异有统计学意义(P0.05)。观察组有效率为93.75%(60/64),高于对照组的75.00%(48/64),差异有统计学意义(P0.05)。结论:消疽汤口服联合活血通络汤熏洗治疗气虚血瘀型糖尿病足可显著提高临床疗效,缓解患者临床症状,改善神经传导速度及踝肱指数,且无不良反应。

Clinical Study on Oral Administration Xiaoju Soup Combined with Huoxue Tongluo Decoction Fumigation in the Treatment of Qi Deficiency and Blood Stasis Type of Diabetic Foot

Objective:To observe the clinical efficacy of oral administration Xiaoju soup combined with Huoxue Tongluo decoction fumigation in the treatment of Qi deficiency and blood stasis type of diabetic foot.Methods:128 patients with Qi deficiency and blood stasis type of diabetic foot patients were randomly divided into two groups,each of 64 cases.All the patients received conventional treatment of diabetes,and on this basis,the control group was given aspirin,insulin,gentamicin and anisodamine,and the observation group was treated with oral administration Xiaoju soup combined with Huoxue Tongluo decoction fumigation.The clinical symptoms,nerve conductive velocity,ankle brachial index and adverse reactions of two groups before and after treatment were compared,and clinical efficacy of two groups were evaluated.Results:After treatment,the TCM syndrome scores of observation group was (8.25 ± 2.33) points,which was lower than (13.58 ± 2.24) points of control group (P ＜ 0.05),and the median nerve,common peroneal nerve motor nerve conduction velocity were (46.57 ± 3.84) m · s-1,(43.02 ± 2.71) m · s-1 respectively,which were higher than (42.58 ± 2.78) m · s-1,(39.58 ± 4.12) m · s-1 in the control group (P ＜ 0.05).And the median nerve,common peroneal nerve sensory nerve conduction velocity were (43.02 ± 2.84) m · s-1,(40.82 ± 3.42) m · s-1 respectively,which were higher than (39.88 ± 2.21) m · s-1,(35.58 ± 3.38) m · s-1 in the control group (P ＜ 0.05).After treatment,ankle brachial index in the observation group was (0.98 ± 0.29),significantly higher than the control group (0.79 ± 0.21),the difference was statistically significant (P ＜ 0.05).The effective rate of observation group was 93.75 ％ (60/64),which was higher than 75.00％ (48/64) of control group (P ＜ 0.05).Conclusion:Oral administration Xiaoju soup combined with Huoxue Tongluo decoction fumigation in the treatment of Qi deficiency and blood stasis type of diabetic foot can significantly increase clini cal efficacy,relieve clinical symptoms,improve nerve conductive velocity and ankle brachial index,and no adverse reactions.
