海南省高血压患者治疗费用及影响因素分析

目的:综合分析海南省高血压患者治疗费用的构成,为合理的配置医疗资源提供科学依据。方法:基于“卫生费用核算体系2011”的核算框架,得出2017年海南省高血压治疗费用总量、人群分布及医疗机构流向等,采用多元线性回归模型研究高血压住院费用的主要影响因素。结果:2017年海南省高血压治疗总费用为105677.48万元,其中93.42%流向在高级别医疗机构,而基层医疗机仅占5.51%。中老年高血压患者治疗费用占比最高。影响住院费用的主要因素依次为住院天数、医疗机构类型、手术情况及患者年龄。结论:海南省高血压医疗负担重,高血压治疗费用机构流向不合理,应增强基层医疗机构防治慢病的作用,加强对中老年人群的防控,减少住院时间能有效控制治疗费用。

Analysis on the Treatment Cost and Influencing Factors of Hypertension Patients in Hainan Province

Objective:To analyze the composition of treatment cost of hypertension patients in Hainan,and to provide scientific basis for reasonable allocation of medical resources.Methods:Based on the accounting framework of“Health Accounting System 2011”,the total cost,population distribution and medical institution flow of hypertension treatment in Hainan in 2017 are obtained,and the main influencing factors of hypertension hospitalization cost are studied by multiple linear regression model.Results:In 2017,the total cost of hypertension treatment in Hainan is 1056.7748 million yuan,93.42%of which went to high-level medical institutions,while only 5.51%went to primary medical institutions.The treatment cost of middle-aged and elderly patients with hypertension accounted for the highest proportion.The main factors influencing the hospitalization expenses are the length of stay,the type of medical institution,the condition of operation and the age of patients.Conclusion:The medical burden of hypertension in Hainan Province is heavy,and the flow direction of the institutions for the treatment of hypertension is unreasonable.It needs to strengthen the role of basic medical institutions in the prevention and treatment of hypertension;enhance the prevention and control of the elderly and reduce hospitalization time,so as to effectively control treatment cost.
