全胸腔镜肺叶切除术的学习体会(附169例报告)

目的总结全胸腔镜肺叶切除术操作步骤及技巧的学习体会。方法对2011年11月—2012年5月,在瑞金医院胸外科学习期间参加169例全胸腔镜下肺叶切除术的操作步骤进行详细总结。结果全组手术顺利,无手术死亡患者,中转开胸小切口辅助13例,中转开胸率7.69%;手术时间70~139 min,平均117 min;手术出血50~1 200 mL,平均250 mL。结论全胸腔镜肺叶切除手术操作难度大,严格掌握手术的适应症,有传统的开放解剖性肺叶切除手术经验,借鉴成熟的手术操作程序,可缩短学习曲线。

Learning experiences of 169 cases of total thoracoscopic lobectomy

Objective To summarize operation procedure and learning experience of total thoracoscopic lobectomy.Methods During the study in the department of chest surgery of Ruijin Hospital,from November 2011 to May 2012,the operation procedures of 169 patients underwent total thoracoscopic lobectomy were summarized carefully.Results All procedures were carried out successfully.There was no dead case during the operation period.There were 13 cases(7.69%) of conversion to open thoracotomy.The surgical duration was 70 to 139 min with the average of 117 min,and the bleeding amount was 50 to 1 200 mL with the average of 250 mL.Conclusion Grasping the core technique of total thoracoscopic lobectomy can ensure the success operation procedures and shorten the learning curve.
