福建省首次发现米克戴德军团菌感染病例

目的 对1例重症肺炎患者进行军团菌病确诊和血清分型,为该血清型军团菌病的诊疗提供科学依据。方法 对病例开展流行病学个案调查和临床病案分析;采集病例标本进行血清抗体检测和常规分离鉴定,对菌株进行生化试验和血清分型;运用PCR技术进行分子诊断。结果 患者2份血清军团菌抗体均为阳性,军团菌培养阳性,生化特性与军团菌相符;菌株与非嗜肺军团菌乳胶凝集阳性,与米克戴德军团菌诊断血清产生凝集。结论 该患者被确诊为感染米克戴德军团菌,为我省首次发现该血清型感染病例;对免疫功能低下且原因不明的肺炎病例应尽早开展军团菌的相关检测,为合理使用抗生素提供依据。

Identification of the first case of Legionella micdadei infection in Fujian Province

We made a definite diagnosis and serotyping on a Legionella disease, in order to provide scientific basis for the diagnosis and treatment of such serotype. An epidemiological investigation and medical records analysis were carried out, and then the samples of the case were collected for the detection of serum antibody, isolation and identification. The isolated strain was carried on biochemical test, serotype and molecular diagnosis by Polymerase chain reaction. Two samples of serum antibody and the culture of Legionella were positive; the characteristics of biochemical test corresponded with Legionella; the isolate coagulated with the non-pneumophila Legionella latex agglutination and diagnostic serum of Legionella micdadei. The patient was definitely diagnosed to be Legionella micdadei disease, which was found in Fujian Province for the first time. The cases of pneumonia with unknown origin and low immunity should be carried on related detection of Legionella as soon as possible, so as to provide evidence for the use of antibiotics.
