胸腰段无神经损伤型爆裂骨折治疗方式的比较

摘    要:目的:应用对照研究的方法来比较、评估保守治疗、外科手术治疗无神经系统损伤的胸腰段前中柱骨折。方法:1999年3月~2002年7月本院收治胸腰段(T11-L2)无神经系统损伤的胸腰段前中柱骨折患者,随机分为手术组(A)、保守组(B)共同34例,获随访28例。其中手术组11例,保守组17例,随访比较2年。结果:通过受伤椎体及神经系统观察提示:保守组(B)无良好复位,手术组(A)复位良好,差异显著;保守组(B)后期多出现神经症状,手术组(A)无神经症状出现,有显著差异。结论:无神经系统损伤的胸腰段前中柱骨折应早期积极手术治疗。

Abstract:Objective:Using control study to compare conservative and operative therapy for the anteromedial column fractures of thoracolumbar spine with no nervous lesion.Methods:Thirty-four cases with no nervous lesion that were diagnosed with the anteromedial column fractures of thoracolumbar spine,including T11-L2 from March 1999 to July 2002.28 cases were followed up for two years,including 11 in Group A as operative theray group,17 in group B as conservative theray group.Results:It was significant differences could be obviously researched via scatching vertebrae and nervous lesion on X-ray and CT.Group A had an excellent reduction,group B worse,in advanced stage,most B had nervous syndromes and A had not them.Conclusion:The anteromedial column fractures of thoracolumbar spine with no nervous lesion should be operated actively in the early time.spinal fractures;spinal cord injury;thoracic vertebrae;lumber vertebrae
