动静理论下的游戏式功能锻炼对肱骨髁上骨折术后患儿肘关节功能及自我效能的影响

摘    要:目的观察动静理论下的游戏式功能锻炼在小儿肱骨髁上骨折术后康复中的作用。方法选取135例肱骨髁上骨折术后患儿为研究对象,随机分为对照组和观察组,对照组67例术后给予常规关节松动训练,观察组68例给予动静理论下的游戏式功能锻炼。干预3个月后,对比2组患儿肘关节功能恢复优良率、肘关节功能采用Mayo评分(包括疼痛、肿胀、关节活动范围和生活自理能力评分)评定]及自我效能采用自我效能量表评价结果期望量表(OE-16)和自我效能期望量表(EE-16)评定]。结果干预3个月后观察组肘关节功能恢复优良率达到89.7%,明显高于对照组的76.1%(P<0.05)。干预3个月后观察组肘关节功能各项Mayo评分明显低于对照组(P<0.05或P<0.001)。干预3个月后观察组自我效能评分明显高于对照组(均P<0.001)。结论动静理论下的游戏式功能锻炼能有效改善肱骨髁上骨折术后患儿的肘关节功能,提高自我效能。

Abstract:Objective To observe the role of play-based functional exercises based on dynamic and static theories in postoperative rehabilitation of children with supracondylar humeral fracture.Methods A total of 135 children who underwent operation for supracondylar humeral fracture were randomly assigned to receive either routine loose joint training(control group,67 cases)or play-based functional exercises based on dynamic and static theories(observation group,68 cases).After 3 months of intervention,the rate of good elbow function recovery,elbow joint function(to evaluate by using the Mayo scoring system,including the pain,swelling,range of joint activity,and self-care ability of life scores)and self-efficacyto evaluate by using the self-efficacy scale evaluation result expectation scale(OE-16)and self-efficacy expectation scale(EE-16)]were compared between the two groups.Results Compared with the control group,play-based functional exercises improved the rate of good elbow function recovery(89.7%vs 76.1%),reduced the Mayo scores,and increased the self-efficacy scores after intervention for 3 months(P<0.05 or P<0.001).Conclusion Play-based functional exercises based on dynamic and static theories can effectively improve elbow function and self-efficacy in children with supracondylar humeral fracture.
