带状疱疹和神经痛

带状疱疹由潜伏在神经节内的疱疹病毒复苏复制而引起，疼痛可以迁延成带状疱疹后遗神经痛。临床表现为受损神经支配的皮肤痛觉过敏、感觉减退、自发性烧灼样痛或电击痛。免疫力低下的患者带状疱疹易迁延为带状疱疹后遗神经痛。带状疱疹神经痛的治疗包括抗病毒治疗、镇痛治疗以及提高免疫力治疗。带状疱疹后遗神经痛治的疗包括重视早期带状疱疹神经痛治疗、药物治疗和介入治疗。

Herper zoster and neuropathic pain

Herper zoster results from reactivation of varicella zoster virus which is latent in the sensory neurons of dorsal root, cranial nerves and autonomic nervous ganglia. Postherpetic neuralgia (PHN) is a kind of neuropathic pain which is caused by viral injury in nervous system and could last for more than 3 months or even longer especially in immuno-compromised patients. Apart from shingles corresponding to the skin area served by the affected sensory spinal or cranial nerve, the manifestations are parathesias, hyper-sensitivity, allodynia, itching or burning to severe lancinating pain. Management of herper zoster includes antiviral therapy, pain management and immuno-modulation. The approaches of controlling PHN are pain killers and interventional therapies.
