血管内皮生长因子和凋亡抑制基因在肝细胞癌中的表达及与预后的相关性

摘    要:目的:探讨血管内皮生长因子(VEGF)和凋亡抑制基因Bag-1、Bcl-2在肝癌细胞、癌旁及正常肝细胞中的表达,以及其与肝细胞癌预后的相关性。方法利用免疫组织化学染色法分别检测60例癌组织、30例癌旁组织及14例正常肝组织中VEGF、Bag-1及Bcl-2的水平,结合临床病理特征及预后对所得数据进行统计学分析。结果 VEGF、Bag-1及Bcl-2在肝癌组织中的表达显著高于癌旁和正常肝组织(P均<0.05);VEGF、Bag-1及Bcl-2的表达与肝癌的分期、有无淋巴结转移、有无癌栓形成以及有无包膜形成相关,差异均具有统计学意义(P均<0.05);与患者性别、年龄、肿瘤分级及大小以及甲胎蛋白(AFP)等因素无关。VEGF、Bag-1及Bcl-2的表达呈高度一致性(P<0.05);VEGF、Bag-1及Bcl-2阳性患者的1年和2年生存率均显著低于阴性患者(P<0.05)。结论 VEGF、Bag-1及Bcl-2与肝癌的生物学行为密切相关,并可作为肝细胞癌患者预后的预测因素。

Abstract:Objective To study the expression of vascular endothelial growth factor (VEGF), Bag-1 and Bcl-2 in hepatocellular carcinoma, para-tumor and normal liver tissue and to investigate the correlation of the presence of VEGF, Bag-1 and Bcl-2 with the prognosis of hepatocellular carcinoma. Methods The expressions of VEGF, Bag-1 and Bcl-2 proteins were detected by immunohistochemical stain in primary liver cancer (PLC) (60 cases), para-cancer (30 cases) and normal liver tissue samples (14 cases). Combined with the clinical pathological characteristics and prognosis of the data were statistically analyzed. Results Immunohistochemical analysis revealed that expression rates of VEGF, Bag-1 and Bcl-2 in PLC tissues were signiifcantly higher than that in para-cancer tissues and normal liver tissues (all P〈0.05). The presence of VEGF, Bag-1 and Bcl-2 in the cancer cells were associated with clinical stage, with or without metastasis in lymph node, with or without capsual in tumor, with or without tumor embolus, all with signiifcant differences (all P〈0.05). However, there was no signiifcant difference with gender, age, tumor differentiation and size, and AFP (all P 〉 0.05). The expression of VEGF, Bag-1 and Bcl-2 had high degree of consistency. The 1-year and 2-year survival rates in patients with VEGF, Bag-1 and Bcl-2 positive were signiifcantly decreased compared with patients of VEGF, Bag-1 and Bcl-2 negative (P〈0.05). Conclusions There was a closely relationship between VEGF, Bag-1, Bcl-2 and clinicopathological characteristics of PLC, and it may serve a prognostic factor for PLC.
