血尿酸水平对2型糖尿病患者周围神经病变诊断的意义

目的探讨2型糖尿病(T2DM)患者血尿酸(SUA)水平与周围神经病变(PNP)的相关性。方法 120例T2DM患者根据是否伴有PNP分为观察组(T2DM伴PNP组)63例和对照组(单纯T2DM组)57例,比较2组患者的空腹血糖(FBG)、糖化血红蛋白(HbA1c)、三酰甘油(TG)、总胆固醇(TC)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)、C-反应蛋白(CRP)、白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)、红细胞沉降率(ESR)、神经功能缺陷量表(NDS)评分及SUA水平,并对T2DM患者的SUA水平与NDS评分的相关性进行分析。结果 2组患者TG、TC、HDL-C、LDL-C、CRP、ESR、IL-6、TNF-α及HbA1c水平比较差异均无统计学意义(P>0.05)。观察组患者FBG和SUA水平及NDS评分显著高于对照组,差异均有统计学意义(P<0.01,P<0.05)。SUA水平与NDS评分呈正相关(P<0.01)。结论 SUA水平对T2DM患者PNP的早期诊断有重要意义。

Significance of serum uric acid levels in the diagnosis of peripheral neuropathy in patients with type 2 diabetes mellitus

Objective To observe the correlation between serum uric acid（ SUA） level and peripheral neuropathy（ PNP） in patients with type 2 diabetes mellitus（ T2DM）. Methods A total of 120 patients with T2DM were divided into observe group（ T2DM with PNP group,n = 63） and control group（ simple T2DM group,n = 57） according to whether complicated with PNP. The levels of fasting blood glucose（ FBG）,glycosylated hemoglobin（ HbA1c）,triacylglycerol（ TG）,total cholesterol（ TC）,high densitylipoprotein-cholesterol（ HDL-C）,low density lipoprotein-cholesterol（ LDL-C）,C-reactive protein（ CRP）, interleukin-6（ IL-6）,tumor necrosis factor-α（ TNF-α）,erythrocyte sedimentation rate（ ESR）,neurological deficit score（ NDS） and SUA were compared between the two groups. The correlation between SUA level and NDS in patients with T2DM was analyzed. Results There was no significant difference in the level of TG,TC,HDL-C,LDL-C,CRP,ESR,IL-6,TNF-α and HbA1c between the two groups（ P〈0. 05）. The levels of FBG,SUA and NDS score in observation group were significantly higher than those in control group（ P〈0. 01,P〈0. 05）. There was positive correlation between SUA level and NDS score（ P〈0. 01）. Conclusion SUA has important meaning in the diagnosis of PNP in patients with T2DM.
