胃黏膜良恶性病变中增殖和凋亡的关系研究

目的 :探讨细胞凋亡和增殖在胃癌发生发展过程中的作用。方法 :对 115例胃癌及癌前病变患者和正常人采用原位末端标记法 (TUNEL)及免疫组化技术检测胃黏膜上皮细胞凋亡和增殖。结果 :正常人、浅表性胃炎、萎缩性胃炎伴肠上皮化生、不典型增生、胃癌患者的细胞凋亡指数分别为 (4 .6 2± 1.87) %、(10 .2 1± 2 .4 9) %、(2 4 .6 3± 35 .19) %、(19.32± 4 .0 7) %、(19.86± 4 .6 1) %。PCNA指数分别为 (3.9l± 1.0 4 ) %、(13.5 2± 4 .71) %、(2 0 ,2 2±4 .83) %、(39.81± 6 .35 ) %、(5 4 .77± 10 .8) %。胃黏膜癌变过程中 ,起初三个阶段细胞凋亡和PCNA同时增加 ,而至萎缩性胃炎伴肠化生 ,细胞凋亡指数显著减少 ,PCNA指数显著增加。结论 :在胃癌演变过程中细胞凋亡起重要作用 ,PCNA是反映胃癌细胞增殖活性的一项重要指标

Relationship between the cell apoptosis and proliferation in gastric cancer and its precancerous lesions

Objective: To investigate the role of apoptosis and proliferation in the pathogenesis of gastric carcinoma. Methods: Using TUNEL technique and immunohistochemistry methods, the cells apoptosis and PCNA indexes were examined in 115 gastric cancer and its precancerous lesions.Results: In the normal group and the patients of chronic superficial gastritis, atrophic gastritis and intestinal metaplasia, gastric dysplasia and gastric carcinomas, their apoptosis indexes were respectively (4.62±1.87)%, (10.21±2.49)%, (24.63±35.19)%, (19.32±4.07)%, (19.86± 4.61)%; PCNA indexes were respectively (3.91±1.04)%, (13.52±4.71)%,(20.22±4.83)%, (39.81±6.35)%, (54.774±10.8)%. the apoptosis and proliferating indexes are increased simultaneously in normal subjects, chronic superficial gastritis, atrophic gastritis and intestinal metaplasia; However, apoptosis indexes are decreased, but PCNA indexes are increased continuously in dysplasia and carcinoma. Conclusion: The present results indicate that apoptosis play an important role in the pathogenesis of gastric carcinoma. PCNA is an important marker to refleat grastric cancer cell proliferation.
