老年不稳定性心绞痛应用普伐他汀钠的疗效与安全性探讨

目的 :观察老年不稳定性心绞痛 (UA )应用普伐他汀钠的疗效与安全性。方法 :全组 6 0岁以上血胆固醇增高患者 80例 ,随机分为治疗组 4 0例和常规对照组 4 0例。对照组给予常规治疗 ,如β-受体阻滞剂、钙通道阻滞剂、硝酸酯类等药物 ,治疗组在常规治疗基础上加用普伐他汀 10 mg,每晚睡前服用 1次。治疗后随访观察记录心脑血管事件的发生率以及药物不良反应 ;比较血脂、C-反应蛋白 (CRP)、纤维蛋白原 (Fg)的变化。结果 :治疗组非致死性心肌梗死发生率 (10 % )显著低于对照组 (2 7.5 % )(P <0 .0 5 ) ;冠心病病死率 (5 % )显著低于对照组 (17.5 % ) (P <0 .0 5 ) ;脑卒中发生率 (2 .5 % )显著低于对照组 (15 .0 % ) (P<0 .0 5 ) ,CRP、Fg下降 ,血浆总胆固醇 (TC)下降并维持于正常水平。不良反应少。结论 :老年 U A服用普伐他汀钠是安全有效的。

STUDY ON EFFECTS AND SAFETY OF PRAVASTATIN FOR ELDERLY UNSTABLE ANGINA PECTORIS

Objective:To observe the effect and safety of pravastatin for elder unstable angia pectoris.Methods:80 patients over 60 years old with hypercholesterolemia were randomly devided into pravastatin therapeutic group(40 cases) and conventional therapeutic group(40 cases). Myocardial infarction incidence, mortality rate of coronary desease, strike incidence and side-effect were assessed. The levels of fibrinogen and fibrin degradation products (Fg).C-reactive protein(CRP), plasma total cholesterol(TC),high density lipoprotein cholesterol(HDL-C), low density lipoprotein cholesterol(LDL-C) were assayed before and after the treatment in two groups.Result:The results show that unfatal myocardial infarction incidence, mortality rate or coronary desease and stroke incidence were,significantly deceased in the therapeutic group(P<0.05).The level of plasma CRP and Fg were markedly reduced after the treatment.Total cholesterolemia deceased and remained normal level and the side-effect was slight.Conclusion:The treatment elder patients with unstable angina pectoris with pravastatin is effective and safe.
