老年人上消化道出血118例临床分析

目的:探讨老年人上消化道出血的临床特点及治疗方法。方法:对118例老年上消化道出血患者按性别分成女性组26例、男性组92例进行回顾性比较分析。结果:药物及饮酒不当饮食是男性上消化道出血的两大主要诱因,老年女性以药物诱因比例最大;消化性溃疡及急性糜烂性出血性胃炎(包括应激相关胃粘膜损伤)仍为主要病因,老年女性以消化性溃疡为第一病因,而老年男性则以急性糜烂性出血性胃炎(包括应激相关胃粘膜损伤)为第一病因;出血量男性组普遍较女性组大,疗效女性组优于男性组。结论:老年男性上消化道出血发病率比女性高,约3.5:1,出血量较大,病情较老年女性急,并发症多,死亡率高,治疗难度大,疗效相对较女性差,需积高度重视。

Clinical Analysis of Upper Gastrointestinal Bleeding of 118 Cases Aged

Objective:To study the clinical features and treatment methods of upper gastrointestinal bleeding in the aged.Methods: 118 elderly patients with upper gastrointestinal bleeding divided by sex,female group 26 cases,male group 92 cases,were retrospective comparative analysis.Results:Drugs,alcohol and improper diet were the two major incentives of male patients with upper gastrointestinal bleeding,the largest proportion of incentives for older women was drug.Major cause were bleeding peptic ulcer and acute erosive gastritis (including stress-related mucosal injury),the first cause in older women was peptic ulcer,and in older men was acute erosive hemorrhagic gastritis(including stress-related mucosal damage).The bleeding of male group was generally larger than female group,efficacy of female group better than male group.Conclusion:The incidence of upper gastrointestinal bleeding of older men than females,about 3.5:1,large amount of bleeding,acute illness than older women,more complications,high mortality,treatment was difficult,treatment is relatively poor than women,need to attach great importance.
