内镜下鼻胆管引流术治疗75岁以上重症胆管炎患者的安全性探讨

目的：探讨经内镜鼻胆管引流术（ENBD）在老年重症胆管炎治疗中的安全性。方法：对2007年5月—2011年5月经ENBD诊治的重症胆管炎病例共42例进行回顾性分析。按年龄将其分为65-75岁年龄组和75岁以上年龄组,比较两组ENBD操作成功率及术后并发症发生率。结果：65-75岁年龄组ENBD操作成功率90.0%（18/20）,75岁以上年龄组成功率86.4%（19/22）,两组比较差异无统计学意义（P〉0.05）。并发症发生率65-75岁年龄组为3.7%,75岁以上年龄组为4.8%,两组比较差异无统计学意义（P〉0.05）。但75岁以上年龄组有1例患者死亡。结论：高龄对ENBD操作成功率及操作相关并发症发生率并无明显影响,同时有心、肺、脑功能并发症时ENBD操作仍有较大风险。

The investigate on the security of endoscopic nasobiliary drainage to people over 75 years with acute cholangitis severe type

Objective：To evaluate the security of endoscopic nasobiliary drainage（ENBD） in the elderly with acute cholangitis severe type.Methods：Retrospectively analyzed the 42 cases who were diagnosed with acute cholangitis severe type in our hospital from May 2007 to May 2011.Divided into two groups,one was 65-75 years old and the other was over 75 years old,compared their operation success rate and the post operative complications of ENBD.Results：The operation success rate of two groups was 90.0%（18/20）,86.4%（19/22）.There was no significance difference（P〉0.05）.The complications was 3.7%,4.8% respectively.There was no significance difference（P〉0.05）.But one patient of the group over 75 years old died.Conclusion：Advanced age is no influence to the operation success rate and complications of ENBD,but the patient who has heart lungs brain dysfunction endure ENBD still has more risk.
