腹腔镜下阴式大子宫切除的临床分析

目的观察腹腔镜辅助下阴式子宫切除术（Laparoscopic assisted transvaginal hysterect omy，LAVH）与腹式子宫切除术（Total abdomin hysterectomy，TAH）进行大子宫切除（子宫≥12孕周）的临床效果。方法对56例子宫增大如12～16孕周，子宫肌瘤、子宫腺肌症等妇科良性疾病患者，行腹腔镜辅助阴式子宫切除术。并随机抽取行TAH者64例作为对照，比较手术时间、术中出血量、术后最高体温、肛门排气时间、术中及术后并发症。结果LAVH组和TAH组相比，手术时间明显延长，术中出血量明显减少（P〈0．05），术后最高体温较TAH组降低，肛门排气时间减短，有统计学意义（P〈0．05）。结论对超过12孕周的子宫进行腹腔镜辅助的阴式子宫切，安全，可行，但需熟练掌握手术技巧。

Laparoscopic Vaginal Hysterectomy Clinical Analysis of Large

Objective To compare the clinical effects of laparoscopy-assisted hyterectomy（LAYH） and through vaginal hysterectomy（TAH） in treatment enlarged uterus （≥12gestation weeks）.Methods The clinical data of cases with enlarged uterus who had undergone uterus hysterectomy in the first affiliated Hospital of Xinxiang medical were collected .56 cases were treated by L AVH and 64 cases by TAH. The operation time ,blood loss in the operation, body temperature and passage of gas by anus after the operation we re analyzed and compared between the two groups. Results The operation time were increased obviously in LAVH＇group compare with those in the TAH group. Bat the blood loss in the operation, body temperature and passage of gas by anus after the operation were decreased singnificiently in LAVH group compare with those in the TAH group. Conclusion Laparoscopically assisted vaginal （enlagrged uterus） is effective, safe and feasible.
