新生儿转运中血糖监测和处理对住院血糖变化的影响

摘    要:目的探讨新生儿转运中的血糖监测和处理对住院血糖的影响。方法采用回顾性分析的方法,对比分析2010年(转运途中未进行血糖监测和处理,设为A组)和2011年(在转运途中纳入血糖监测和处理,设为B组)患儿在转运中低血糖或高血糖的发生率;入院后血糖异常的发生率;血糖恢复到正常的时间和相关临床症状消失时间。结果 A组1 122例转运患儿入院后低血糖发生率为11.2%,高血糖发生率为7.6%;B组1 032例转运中低血糖发生率为10.5%,高血糖发生率为6.9%,入院后监测低血糖和高血糖的发生率分别是5.8%和3.2%。A组低血糖恢复到正常的时间为(2.5±2.2)d,高血糖恢复到正常的时间为(3.2±1.5)d,而B组低血糖恢复到正常的时间为(1.7±1.4)d,高血糖恢复到正常的时间为(1.7±1.1)d,A、B两组差异有统计学意义(均P<0.05)。结论转运中血糖异常发生率很高,监测和处理有助于降低新生儿血糖异常的发生率,及早维持患儿血糖在正常范围内。

Effect of Monitoring and Management of Blood Sugar during Neonatal Transportation on Hospitalization Prognosis

Abstract:Objective To explore the effects of blood sugar management during neonatal transportation on the blood sugar level after admission.Methods Patients were divided retrospectively into two groups.There were 1 122 patients not monitoring blood sugar during transportation in group A,while blood sugar management during transportation was taken in group B of 1 032 patients.Blood sugar was detected at the time of admission and during the hospital stay.The incidence of hyperglycemia/hypoglycemia,and time of blood sugar restoration to normal level and time of symptoms disappearance were investigated.Results Incidence of hyperglycemia and hypoglycemia was 7.6% and 11.2% in group A.Incidence of hyperglycemia and hypoglycemia was 6.9% and 10.5% in group B before admission,and 3.2% and 5.8% after admission.Time of blood sugar restoration to normal level was(2.5±2.2)days from hypoglycemia and(3.2±1.5)days from hyperglycemia in group A,and that was(1.7±1.4)days and(1.7±1.1)days in group B,respectively.Conclusion Incidence of hyperglycemia/hypoglycemia is very high during transportation.Blood sugar management during neonatal transportation contributes to reduction of incidence of abnormal blood sugar as well as earlier restoration to the normal blood sugar.
