小剂量米索前列醇联合利凡诺在瘢痕子宫妊娠引产中的应用效果

目的：研究小剂量米索前列醇结合利凡诺注射在瘢痕子宫妊娠引产中的应用效果。方法将2014年8月至2015年9月在宝鸡市中心医院就诊的58例瘢痕子宫妊娠患者根据引产方法随机分组，研究组29例采用小剂量米索前列醇结合利凡诺注射；对照组29例仅用利凡诺注射引产，对比两组引产效果。结果研究组引产成功率为96．55％，显著高于对照组的79．31％（χ2＝4．06，P＜0．05）；研究组的引产时间（5．11±2．23）h和总出血量（137．54±10．36）mL均显著低于对照组（t值分别为7．153和6．361，均P＜0．05）；研究组有2例患者出现发热不良反应，不良反应发生率为6．90％；对照组有8例患者出现不良反应，其中5例的临床症状表现为发热，1例出现大出血，2例出现软产道裂伤，不良反应发生率为17．24％。研究组的不良反发生率低于对照组，差异具有统计学意义（χ2＝5．873，P＜0．05）。结论采用小剂量米索前列醇结合利凡诺注射能有效提高瘢痕子宫产妇引产成功率，且用药过程中安全性较高，对患者身体的危害性较小。

Application of small dose of misoprostol combined with rivanol injection in scar uterine pregnancy labor induction

Objective To study the application effect of small dose of misoprostol combined with rivanol injection on scar uterine pregnancy labor induction .Methods Totally 58 cases of uterine scar pregnancy patients in Baoji Town Center Hospital were divided into two groups according to approach of induced labor , 29 cases in study group with small dose of misoprostol combined with rivanol injection and 29 cases in control group only with injection of rivanol .Induction effect was compared between two groups .Results The success rate of induced labor of the study group was 96.55%, which was obviously higher than that of the control group (79.31%) (χ2 =4.06, P<0.05).The labor induction time and total amount of bleeding in the study group were 5.11+2.23h and 137.54 ±10.36ml, respectively, which were lower those in the control group (t value was 7.153 and 6.361, respectively, both P<0.05).Two cases in the study group had adverse reaction of fever , and the incidence was 6.90%.There were 8 cases in the control group with adverse reaction and the incidence was 17.24%, including 5 cases of fever, 1 case of hemorrhage and 2 cases of soft birth canal laceration .The adverse reaction rate of the study group was significantly lower than that of the control group (χ2 =5.873, P<0.05).Conclusion Using small dose of misoprostol combined with rivanol injection can significantly improve the success rate of scar uterine pregnancy induced labor .It also has higher medication safety and less harm on patients .
