不孕及流产妇女实验室常用检查项目的应用价值分析

目的:分析不孕及流产妇女实验室常用检查项目的应用价值。方法:采用间接血凝法检测2085例不孕妇女及352例自然流产妇女的抗精子抗体(ASAb);用酶联免疫吸附实验(ELISA)检测血清抗子宫内膜抗体(EMAb)、血清抗卵巢抗体(AOAb)、血清抗心磷脂抗体(ACA)、弓形虫(TOX)、单纯疱疹Ⅰ型病毒(HSV-I)、风疹病毒(RUV)及巨细胞病毒(CMV)的IgG抗体;采用一步免疫层析法检测沙眼衣原体(CT);培养法检测解脲支原体(UU)。结果:不孕组中血清ASAb、宫颈粘液ASAb、血清中EMAb、AOAb、ACA、TOX、HSV-I、RUV、CMV、尿道UU、宫颈UU、CT的阳性率分别为1.94%、50.82%、22.88%、17.09%、17.39%、9.68%、13.38%、40.51%、36.08%、58.27%、43.97%、55.57%;流产组中上述指标的阳性率分别为1.23%、44.51%、17.40%、18.42%、19.58%、10.87%、17.89%、28.42%、23.16%、58.99%、39.43%、56.66%,其中流产组中的宫颈粘液ASAb、血清中EMAb、CMV的阳性率均与不孕组中相对应指标的阳性率有明显的差异,并且不孕组和流产组中的宫颈粘液ASAb的阳性率均较相对的血清ASAb的阳性率高。结论:ASAb、EMAb、AOAb、ACA、TOX、HSV-I、RUV、CMV、UU、CT在不孕及自然流产妇女中有较高的检出率,上述项目的应用对提高治疗水平和优生优育有重要价值,应成为相关就诊者的常规检查项目。

The applied value of laboratory routine examination items in infertility and spontaneous abortion

Objective:To study the applied value of laboratory routine examination items in infertility and spontaneous abortion.Methods:Antisperm antibodies(ASAb)were measured and analyzed in 2 085 infertility women and 352 spontaneous abortions by indirect haemagglutnation;The IgG antibodies of antiendometrial antibodies(EMAb),antiovarian antibodies(AOAb),anticardiolipin antibodies(ACA),toxoplasma(TOX),herpes simplex virus I(HSV-I),rubella virus(RUV)and cytomegalovirus(CMV)were measured and analyzed by enzyme linked immunosorbent assay(ELISA);The infection of ureaplasma urealyticum(UU)was measured and analyzed by cultivation;Chiamydia trachomatis(CT)was measured and analyzed by one-step-immunochromatographic method.Results:In the infertility group,the positive rates of ASAb in serum and cervix mucus,EMAb,AOAb,ACA,TOX,HSV-I,RUV and CMV in serum,UU in urethra,UU and CT in cervix were 1.94%,50.82%,22.88%,17.09%,17.39%,9.68%,13.38%,40.51%,36.08%,58.27%,43.97%,55.57%;In contract,the items in the abortion group were 1.23%,44.51%,17.40%,18.42%,19.58%,10.87%,17.89%,28.42% and 23.16%,58.99%,39.43%,56.66%.Conclusion:In infertility and spontaneous abortion women,the high detection rate of ASAb,EMAb,AOAb,ACA,UU,CT,TOX,HSV-I,RUV and CMV,which indicates important values for both the medical standard improvement and aristogenesis.As a result they should be included in the routine examination items for the patients.
