右美托咪定在腹腔镜小儿腹股沟疝手术中的应用

目的:观察右美托咪定复合丙泊酚在腹腔镜小儿腹股沟疝手术中的麻醉效果。方法:择期行腹腔镜斜疝手术患儿60例,随机分为两组,右美托咪定复合丙泊酚组(D组)和丙泊酚组(P组),静注丙泊酚2.5mg/kg、芬太尼2.5ug/kg同时吸入4%七氟烷(氧流量4L/min)麻醉诱导,2min后置入喉罩。D组同时泵注右美托咪定0.8ug/kg,输注时间超过10min。记录各时间点MAP、SpO2、HR及手术时间、苏醒时间、不良反应和术后躁动发生率。结果:两组患儿SpO2、苏醒时间无显著性差异;诱导后,与P组相比,D组MAP、HR下降(P0.05);丙泊酚总用量、不良反应及术后躁动发生率D组均低于P组(P0.05)。结论:右美托咪定复合丙泊酚可安全用于腹腔镜小儿腹股沟疝手术,能降低术中不良反应及术后躁动发生率。

Application of Dexmedetomidine Combined with Propofol in Children undergoing Laparoscopic Surgery for indirect Inguinal Hernia

Objective：To investigate the effect of dexmedetomidine combined with propofol in children undergoing laparoscopic herniotomy for indirect inguinal hernia. Methods：Sixty cases undergoing selective laparoscopic indirect inguinal hernia surgery were randomly divided into two groups ：dexmedetomidine combined with propofol group （group D）, propofol group（group P）.All patients were induced with propofol 2.5mg/kg,fentanyl 2.5ug/kg, inhalation of 4% sevoflurane （4L/min flow oxygen）. Group D also received dexmedetomidine 0.8ug/kg（ over five minutes） as part of the premedication regime.MAP,SpO2,HR at the following time point： before administration ,laryngeal mask airway （LMA） insertion,LMA extubation and total dose of propofol were recorded.Time of operation and recovery,adverse re- action,restlessness were recorded too.Results：There were no significant difference in SpO2,time of recovery between two groups.MAP,HR after anesthesia ,the total dose of propofol, the percentage of adverse reaction and restlessness were lower in group D than those of group P （P〈0.05）.Conclusion：Dexmedetomidine combined with propofol during general anesthesia induction can decrease the percentage of adverse reaction and restlessness.
