妊娠期糖尿病162例临床分析

目的:探讨妊娠期糖尿病早期筛查、诊断,及时治疗对母儿预后的影响。方法:回顾性分析我院2005年1月~2006年10月诊治的妊娠期糖尿病患者162例(GDM组)与随机选择同期住院中非妊娠期糖尿病、无内外科并发症的孕妇162例(对照组),就孕产妇并发症、分娩方式、妊娠结局、围产儿情况作比较分析。结果:GDM组患者的剖宫产率明显高于对照组(P<0.01),母儿合并症羊水过多、羊水过少、阴道炎、胎膜早破、妊娠期高血压疾病、酮症酸中毒、胎儿窘迫、新生儿窒息、新生儿呼吸窘迫综合征、死胎、新生儿死亡、早产儿、巨大儿的发病率与对照组相比无显著性差异(P>0.05)。结论:早期筛查、诊断,及时治疗妊娠期糖尿病,适当放宽剖宫产指征可降低母儿并发症。

Clinical analysis of 162 cases with gestational diabetes mellitus

Objective:To investigate the early screening,diagnosis and timely treatment of gestational diabetes mellitus(GDM) and evaluate their effects on the outcome of maternal and neonatal.Methods:To retrospective analyze 162 cases of gestational diabetes mellitus(GDM group) and the random selection of nondiabetic,no surgical and medical complications in pregnant women hospitalized 162 cases in the same period(control group) of our hospital from January 2005 to October 2006 during the diagnosis and treatment.Maternal complications,childbirth mode,the outcome of pregnancy,the prenatal conditions were comparative analyzed.Results:The rate of cesarean section in GDM group was significantly higher than those in the control group(P<0.01),there was no significant difference between GDM group and control group in the incidence of maternal and neonatal complications: polyhydramnios,oligohydramnios,vaginitis,premature rupture of membrane,hypertensive disorder complicating pregnancy,ketoacidosis,fetal distress,neonatal asphyxia,neonatal respiratory distress syndrome,fetal death,neonatal death,premature infant,fetal macrosomia(P>0.05).Conclusion:Early screening,diagnosis and timely treatment of gestational diabetes mellitus,and appropriate relaxation of cesarean section indication can reduce the complications of maternal and neonatal.
