摘    要:目的 探讨掌侧T形接骨板治疗桡骨远端不稳定骨折的疗效。方法 自2003年1月至2006年6月采用掌侧T形接骨板内固定或再加外固定架治疗桡骨远端骨折52例.AO分型A3型13例,B2型5例.B3型7例.C1型10例,C2型12例.C3型5例。其中植骨9例,同时行尺骨内固定7例。结果 52例全部获得随访.随访6~41个月.平均22个月,所有病例骨折愈合良好,无不愈合或延迟愈合。采用改良Mcbride评分和纽约骨科医院腕关节评估标准:优38例,良9例,可5例,优良率为90.4%。结论 掌侧T形接骨板治疗桡骨远端不稳定骨折,内固定稳妥可靠,可早期功能锻炼,疗效满意。

Abstract:Objective To evaluate the efficacy of volar T-shaped plate fixation for unstable fractures of distal radius.Methods From January 2003 to June 2006,52 patients with distal radius fractures had been treated with T- shaped plate fixation via volar approach combined with an external fixator.Fractures were clarified using the AO classification.13 fractures were classified as type 23 A3,5 as 23 B2,7 as 23 B3.10 as 23 C1,12 as 23 C2 and 5 as 23 C3.Autogeneic bone grafting was applied in nine patients,and ipsilateral ulna was simultaneously fixed in 7 patients.Results All patients were followed up for an average of 22 months(range from 6 to 41 months).De- layed union or non-union was not observed.According to the modified Mcbride grading, thirty-eight patients was graded as excellent, nine as good and 5 as fair,the overall accellent or good rates was 90.4 %.Conclusion Volar T- shaped plate fixation combined with external fixation is reliable and effective in treatment of unstable fracture of dis- tal radius,stable fixation can be achieved to allow early joint motion in most cases.
