中西医结合对急性肝衰竭大鼠肠道屏障功能保护作用的研究

摘    要:目的 观察大黄煎液联合谷氨酰胺能否减少急性肝衰竭大鼠肠道细菌易位的发生并探讨其防治作用的机制.方法 SD大鼠随机分成5组,模型组(A组),大黄防治组(B组),谷氨酰胺(Gln)防治组(C组),大黄联合谷氨酰胺防治组(D组),对照组(E组).A、B、C、D组腹腔注射D-氨基半乳糖(GalN)建立急性肝衰竭(AHF)大鼠模型.造模前两天,B组予大黄煎液灌胃,C组予Gln溶液灌胃,D组予大黄煎液和Gln溶液灌胃,A组和E组予生理盐水灌胃作为对照.造模4天后处死动物,观察肠系膜淋巴结细菌易位及平均组织含菌量的情况,观察回肠组织学改变,测量回肠绒毛高度和隐窝深度.结果 D组肠系膜淋巴结易位率(50%)明显低于A组(100%)(P<0.05),D组亦低于B组(63.6%)、C组(60%)(P>0.05).D组肠系膜淋巴结平均组织细菌含量(1.80 ±2.31)×103CFU/g低于A组(10.54 ±4.08)×103CFU/g(P<0.01),但与B组(3.59±3.80)×103CFU/g、C组(3.12±3.54)×103CFU/g比较差异无统计学意义(P>0.05).D组的肠绒毛高度1.77±0.14μm显著高于B组1.23±0.92μm (P<0.01)和C组1.45±0.13μm (P<0.01).D组的肠隐窝深度1.46±0.15μm显著高于B组0.92±0.11 μm(P<0.01)和C组1.15±0.13μm (P <0.01).结论 大黄能护肝降酶退黄,改善肠道微循环,谷氨酰胺具有保护肝脏、维持肠道黏膜屏障功能等作用.大黄联合谷氨酰胺能改善AHF大鼠的肠道黏膜屏障功能,对AHF大鼠肠道细菌易位具有预防作用.

Abstract:Objective To investigate if the combination of rhubarb and glutamine(Gln) can reduce the incidence of the intestinal bacterial translocation as well as mechanism of action in acute hepatic failure(AHF) in rats.Methods A mouse experimental model of AHF was developed by intraperitoneal injection of Galactosamine.rats were randomly divided into 5 groups:the model group (A),prevention and cure of the rhubarb group (B),prevention and cure of the Gln group (C),prevention and cure of the combination of rhubarb and Gln group (D),normal control group (E).Two days before intraperitoneal injection,Tats in group B were fed with Rhenm Officinal Decoction,rats in group C were fed with solution of Gln,rats in group D were fed with Rhenm Officinal Decoction and solution of Gln,rats in group A and E were fed with physiological saline.Four days after operation,the intestinal bacterial transloaction percentage and bacteria in MLN were measured,histological changes in ileal were determined.The villus height,crypt depth of ileum were analyzed.Results The frequency of the bacteria found in MLN was significantly lower in group D (50%) than in group A (100%) (P 〈 0.05),and it was also lower in group D than in group B(63.6%) and group C(60%).The quanlity of the bacteria in MLN tissue was significantly lower in group D (1.80 ± 2.31) × 103 CFU/g than in group A (10.54 ± 4.08) × 103 CFU/g (P 〈 0.01),and it was lower in group D than in group B (3.59 ± 3.80) × 103 CFU/g and group C (3.12 ± 3.54) × 103 CFU/g(P 〉 0.05).The villus height was significantly higher in group D 1.77 ± 0.14μm than in group B 1.23 ± 0.92μm (P 〈 0.01) and group C 1.45 ± 0.13 μm (P 〈 0.01).The crypt depth of the ileum was significantly higher in group D 1.46±0.15μm than in group B0.92±0.11μm (P〈0.01) and group C 1.15 ±0.13μm (P〈0.01).Conclusion The present results show that rhubarb can improve microcirculation of intestinal.Gln can also improve the
