左西孟旦治疗老年慢性心衰失代偿期的疗效评价

摘    要:目的评价血浆N端B型利钠肽前体(NT—proBNP)对心功能的影响及左西孟旦治疗慢性心衰失代偿期的临床疗效。方法选择40例确诊的慢性心衰失代偿期住院患者,随机分为常规治疗组和左西孟旦组各20例。用药前以及开始用药后1个月两组均行NT—proBNP检测、超声心动图测左室射血分数(LVEF),且分别与治疗前诸指标进行比较。结果血浆NT—proBNP水平随着心功能NYHA分级严重程度的递增而增加,各组间均有显著性差异(P〈0.05),血浆NT—proBNP水平与LVEF呈负相关(P〈0.05);左西盂旦能显著降低失代偿心衰患者血浆NT—proBNP水平(P〈0.05)。结论血浆NT—proBNP水平反映心功能受损的敏感指标。左西孟旦能增加失代偿心力衰竭患者的心肌收缩力及改善心功能,显著降低血浆NT—proBNP水平。

Efficacy evaluation of Levosimendan in decompensated chronic heart failure

Abstract:Objective To evaluation the influence of plasma N-terminal B-type natriuretic peptide(NT-proBNP) on cardiac function and the efficacy of levosimendan in the treatment of decompensated chronic heart failure.Methods 40 cases of hospitalized patients diagnosed with decompensated chronic heart failure are randomLy divided into 20 cases of the conventional treatment group and the levosimendan group.The two groups are NT-proBNP testing,echocardiography measurements in LVEF before treatment medication and after a month,and it will be compared with various indicators in the pre-treatment.Results The plasma NT-proBNP levels increased with cardiac function in NYHA functional class severity increments.There are significant differences among the groups(P < 0.05).Plasma NT-proBNP level and LVEF is negatively correlated(P < 0.05).Levosimendan significantly decreases plasma of NT-proBNP level in decompensated heart failure patients(P < 0.05).Conclusion Plasma NT-proBNP level is a sensitive indicator in reflecting cardiac function impairment.Levosimendan can improve cardiac function of patients with decompensated heart failure,significantly reducing the level of plasma NT-proBNP.
