鞍区毛细胞星形细胞瘤影像学表现及病理对照分析

目的：探讨鞍区毛细胞型星形细胞瘤的影像学特征并与病理结果进行对照分析。方法：回顾性分析10例经手术及病理证实的鞍区毛细胞型星形细胞瘤的CT、MRI特征并与病理改变进行对照。结果：10例中实性肿瘤2例，囊实性8例。主要CT表现为不均匀稍高密度，1例边缘见钙化灶。主要MRI表现为TzwI上呈不均匀高信号，T1wI上呈低信号，增强扫描示肿瘤呈明显不均匀强化，其内可见多发小囊状或裂隙样不强化区。10例视交叉显示不清，3例肿瘤包绕大脑中动脉，1例伴有软脑膜播散。结论：鞍区毛细胞型星形细胞瘤多为囊实性且多具有向周围组织侵袭及软脑膜播散的表现，其影像学表现有一定的特征性。

Analysis of imaging manifestations of sellar region pilocytic astrocytoma and correlation with pathology

Objective：To investigate the imaging features of sellar region pilocytic astrocytoma （PA） and correlated with pathology. Methods：The CT, MRI features of 10 cases with pathologically confirmed sellar region PA were analyzed retrospectively,and correlated with clinical pathology. Results： Of the ten cases, there were solid （2 cases） and solid-cystic （8 cases） tumor. The main CT finding was heterogeneous and slightly hyper-attenuated on CT images, peripheral calcifica- tions were seen in one case. The main MRI findings were heterogeneously hyperintense on T2 Wl, hypointense on T1 WI, and obviously heterogeneously enhanced after contrast administration. Multiple microcystic or slit-like non-enhanced region could be found. The optic chiasm was blurred or could not be displayed clearly in ten cases. Middle cerebral artery embedded within tumor could be revealed in three cases. Leptomeningeal metastasis was showed in one case. Conclusion：The common characteristic appearances of PA in sellar region were solid-cystic mass, tending to involve adiacent structures and having leptomeningeal metastasis.
