超声和CT对腹部实质脏器闭合性外伤诊断价值的探讨

摘    要:目的 探索超声和CT对腹部实质脏器闭合性外伤的诊断价值.方法 对经过综合征治疗和手术治疗确诊为腹部实质脏器闭合外伤的87例患者、103个外伤灶的CT和超声表现进行分析和回顾.结果 在103个外伤脏器当中,利用超声进行准确诊断的共有91个(88.34%),利用CT进行准确诊断的有96个(93.20%);在25个对损伤复合脏器的诊断当中,利用超声进行诊断的准确率为88%(22/25),利用CT进行诊断的准确率为100%(25/25).结论 利用CT和超声腹部实质脏器闭合性外伤诊断的价值是不容置疑的,二者进行诊断的准确率都比较高,而且差别不大,在大多数情况下不需要进行联合使用,进行腹部脏器单一损伤检查是应首选超声检查,而对复合损伤进行检查时应首选CT诊断,并且在证实超声诊断时运用CT诊断的方法是不合理的.

Abstract:Objective To explore the ultrasound and CT diagnostic value of solid organ closed abdominal trau- ma. Methods After syndrome treatment for solid organ closed abdominal trauma diagnosis and surgical treatment of 87 cases, 103 patients with trauma, CT and sonographic findings was analyzed and review of the oven. Results Among 103 trauma, viscera, the use of ultrasound for accurate diagnosis of a total of 91, accounting for 88.34%, the use of CT for accurate diagnosis, has 96, accounts for 93.20% ; In 25 of the diagnosis for complex organ damage and accuracy of ultrasonic diagnosis was 88% (22/25), using the CT diagnostic accuracy was 100% (25/25). Conclusion The use of computed tomography (CT) and ultrasound real viscera blunt abdominal trauma diagnosis value is no doubt, the diagnosis accuracy is high, and the difference is not big, in most cases do not need to be combined, the examination of the abdominal viscera damage to a single ultrasound is preferred, and carried out on the composite damage should be preferred when CT examination, and confirmed in the ultrasonic diagnosis using CT diagno- sis method is not reasonable.
