摘    要:目的探讨造影剂对肾功能影响,引起造影剂肾病(CIN)的易患因素及其防治。方法选择接受冠状动脉造影术患者222例,检测术前、术后24h、72h的血尿素氮、血肌酐(cr)。对造影后24~72h血肌酐较原来增高25%或者44μmol/L以上定义为CIN。结果肾功能不全组CIN发生率20%,明显高于术前肾功能正常组的4.7%,差异具有显著性(P〈0.05)。非CIN患者,术中造影剂用量少,而术前补液量明显多于CIN患者,差异具有显著性(P〈0.05)。结论造影剂可引起一过性肾功能改变,CIN与造影剂剂量、补液量密切有关。

Abstract:Objetive To investigate the effect of contrast media on the renal function and the risky factors of contrast-induced nephropathy(CIN) and its prevention measurements.Methods Blood levels of BUN and Cr were measured before operation,24h after operation and 72h after operation in 222 patients who received CAG.CIN was defined as that level of blood Cr was elevated by 20 percent or 44μmol/L compared with basal level.Results Incidence of CIN in group of patients with renal failure was higher than that in group of patients with normal renal function significaltly(20% versus 4.7%,P〈0.05).Dose of contrast media in patients with non CIN was less than that in patients with CIN significantly(P〈0.05).Amount of liquid infusion in patients with non CIN was more than that in patients with CIN significantly(P〈0.05).Conclusion Contrast media could induce transient change of renal function.Incidence of CIN was related to dose of contrast media and amount of liquid infusion.
