以术后快速康复理念为导向无管道置入在达芬奇机器人纵隔手术中应用

目的探讨在术后快速康复(ERAS)理念引导下应用达芬奇机器人手术系统(DVSS)行纵隔手术中无管道置入应用的可行性及临床效果。方法将60例采用DVSS辅助下纵隔疾病手术的患者随机分A组和B组,每组各30例患者。A组患者给予常规手术流程,B组患者采用无管道置入方法完成手术。通过优化手术配合流程,对两组患者术后相关指标进行比较。结果两组患者均顺利完成手术。与A组比较,B组患者术后发生躁动、声音嘶哑、咽喉疼痛、尿路感染的概率以及住院时间等方面,差异均有统计学意义(P<0.05)。结论以ERAS理念为导向的无管道置入的手术方法应用在DVSS纵隔手术中,不仅是安全、有效、经济的,而且可以促进患者术后快速康复,缩短住院时间,提高患者满意度。

Application of no pipeline placement in mediastinal surgery via da Vinci surgical system oriented by ERAS concept

Objective To investigate the feasibility and clinical effect of the application of no pipeline placement in mediastinal surgery using da Vinci surgical system(DVSS) oriented by ERAS concept.Methods Totally 60 cases of patients that received mediastihal surgery assisted by DVSS were randomly divided into the control group and the observed group with 30 cases in each group.In the control group,patients received treatment in routine surgical procedures;while in the observed group,no pipeline placement method was used to complete the operation.By optimizing operative coordination procedure,the postoperative indexes between two groups were compared.Results All the surgeries of both groups were successfully completed.However,the cases of patients with postoperative agitation,the incidence of hoarseness,throat pain and urinary tract infection,as well as the length of hospital stay in the observed group were all significantly decreased than those in the control group (P ＜ 0.05).Conclusion The application of no pipeline placement operation via DVSS method oriented by the concept of ERAS is not only safe,effective and economic,but also can promote rapid recovery after surgery,shorten the hospitalization time and improve patient satisfaction.
