血尿酸升高及氧化应激在冠心病发生中的作用

摘    要:目的通过了解冠心病(CHD)患者血清尿酸(UA)水平、氧化应激损伤程度及其相互联系,探讨血UA升高及氧化应激损伤在CHD发生中的作用。方法选择急性冠脉综合征(ACS)、稳定型心绞痛(SAP)和对照组(CO)患者各30例,测定外周血清UA、丙二醛(MDA)和总超氧化物岐化酶(T—SOD)活力,然后进行统计学分析。结果ACS和SAP组患者的血清UA和MDA含量均高于CO组(P均〈0.05),而T—SOD活力均低于CO组(P均〈0.05);ACS组和SA组相比,血清UA水平差异无统计学意义(P〉0.05),而血清MDA水平明显增高、T—SOD活力则明显降低(P均〈0.05);冠心病患者的UA与MDA呈正相关(r=0.614,P〈0.05),与T—SOD活力呈负相关(r=-0.591,P〈0.05);SAP组高尿酸血症(HUA)者与正常血UA者相比,血清T—SOD活力明显降低(P〈0.05);ACS组HUA者与正常血UA者相比,血清MDA水平明显增高、T—SOD活力明显降低(P均〈0.05)。结论血清UA水平升高可能是CHD的一个危险因素,其可能通过促进氧化应激损伤增加,抗氧化能力降低,导致了CHD的发生发展。

Abstract:ObjectiveTo elucidate the effects on elevated serum uric acid(UA) level as well as oxidative stress damage in the pathogenesis of coronary heart disease(CHD) by exploring the level of serum UA and oxidative stress damage in CHD.MethodsTo select 30 patients with acute coronary syndrome(ACS),30 patients with stable angina pectoris(SAP) and 30 patients as controlled(CO) group,and the levels of serum UA,MDA and T-SOD activity were measured.ResultsThe levels of serum UA and MDA were higher,while T-SOD activity was lower in ACS and SAP group compared with CO group(P<0.05);and the serum MDA level was higher in ACS group compared with SAP group(P<0.05),while T-SOD activity was lower(P<0.05) and there was no difference in the serum UA between two groups(P>0.05).The serum UA level was positively correlated with MDA(r=0.614,P<0.05),but negatively correlated with T-SOD activity(r=-0.591,P<0.05)in CHD patients.Moreover,to SAP group,the serum level of T-SOD activity was lower in the higher serum UA patients compared with the normal serum UA patients(P<0.05);To ACS group,the levels of MDA was higher(P<0.05),while T-SOD activity was lower(P<0.05)in the higher serum UA patients compared with the normal serum UA patients.ConclusionThe elevated serum UA is probably a risk factor of CHD and induces the pathogenesis and development of CHD by promoting the oxidative stress damage and the ability of anti-oxidation decline.
