影响大面积脑梗死预后的相关因素分析

目的分析影响大面积脑梗死预后的相关因素。方法回顾性分析65例大面积脑梗死患者,以梗死后30d作为时间判定标准,分为存活及死亡2组,观测包括年龄、性别、意识水平、梗死半球等12项指标,行单因素分析,判断其与大面积脑梗死预后的关系。结果单因素检验表明,意识水平、同向凝视、合并肺部感染、伴发心脏病、去骨瓣减压术等5个因素为影响大面积脑梗死后30d预后的危险因素(P<0.05),而年龄、性别、梗死半球、合并上消化道出血、伴发高血压病及糖尿病、血中白细胞计数等7个因素对大面积脑梗死后30d预后无影响(P>0.05)。结论对于伴有意识障碍、同向凝视,合并肺部感染、伴发心脏病的大面积脑梗死患者应给予高度重视,积极治疗伴发疾病,必要时需行去骨瓣减压术。

Analysis of the factors affecting prognosis in patients with massive cerebral infarction

Objective To analyze the factors affecting prognosis in patients with massive cerebral infarction.Methods The materials of 65 patients with massive cerebral infarction were retrospectively analyzed,according to survival or dead at 30d after the infarction, the relationship between survival and affecting factors(including age,sex,level of consciousness,hemisphere of infarction,and so on) were analyzed by chi-square test.Results Level of consciousness,gaze palsy,complicating pulmonary infection,heart disease history and hemicraniectomy were factors of affecting survival (P<0.05),but age,sex,hemisphere of infarction,complicating upper gastrointestinal hemorrhage,hypertensive disease,diabetes history and leucocyte count had no effect to survival (P>0.05).Conclusion The conscious disturbance,gaze palsy,complicating pulmonary infection,heart disease history should be pay more attention in patients with massive cerebral infarction.
