妊娠期糖尿病与孕妇铁代谢相关性研究

目的:探讨妊娠期糖尿病(GDM)与孕妇铁代谢水平的相关性。方法:856例孕妇按血糖筛查和糖耐量试验筛选出GDM孕妇26例,随机选择糖耐量正常孕妇26例作为对照组,分别测定两组孕妇的空腹血糖(FPG)、空腹胰岛素(F ins)、糖化血红蛋白(HbA1C)、空腹C肽、血清铁(SI)、血清铁蛋白(SP)、总铁结合力(TIBC)、转铁蛋白饱和度(TS)等指标,计算胰岛素抵抗指数(HOMA-IR)以评价胰岛素抵抗水平,并对两组孕妇的相应指标作对比和相关分析。结果:GDM组孕妇FPG、F ins、HbA1C、HOMA-IR、SI、SF和TS水平显著高于对照组,空腹C肽、TIBC值显著低于对照组。GDM组孕妇血清SF水平与糖化血红蛋白、HOMA-IR值相关。结论:铁代谢与GDM患者胰岛素抵抗密切相关,适当降低铁储备可能减轻胰岛素抵抗。

Analysis on correlation between gestational diabetes mellitus and iron metabolism of pregnant women

Objective:To explore the correlation between gestational diabetes mellitus(GDM) and iron metabolism of pregnant women.Methods:26 pregnant women were diagnosed as GDM among 856 pregnant women by blood glucose screening and glucose tolerance test,and 26 pregnant women with normal glucose tolerance test results were selected as control group.The levels of fasting plasma glucose(FPG),fasting insulin(Fins),glycosylated hemoglobin A1(HbAlc),fasting C peptide(FCP),serum iron(SI),serum ferritin (SF),total iron binding capacity(TIBC) and transferring saturation(Ts) were detected.Homa insulin resistance index(HOMA-IR) was calculated to evaluate the level of insulin resistance,the indexes of two groups were compared.Results:The levels of FPG,Fins,HbAlC,HOMA-IR,SI,SF and Ts of GDM group were significantly higher than those of control group,whereas FCP and TIBC reversed.The level of SF of pregnant women with GDM was related to the levels of HbA lC and HOMA-IR.Conclusion:Iron metabolism is closely related to insulin resistance of GDM women.Decreasing iron reserve properly may help to relieve insulin resistance.
