3D打印技术在复杂髋部疾病全髋关节置换术中的应用

摘    要:目的评价3D打印技术在复杂髋部疾病全髋关节置换手术中应用的临床价值。方法回顾性分析自2016-06—2018-12采用全髋关节置换术治疗的37例(42髋)复杂髋部疾病,19例(22髋)行常规手术(常规组),18例(20髋)采用3D打印技术辅助手术(3D打印组)。比较2组手术时间、术中出血量、并发症发生率,以及末次随访时疼痛VAS评分、髋关节功能Harris评分。结果37例均获得随访,随访时间平均23.5(6~48)个月。3D打印组手术时间较常规组明显缩短,术中出血量较常规组明显减少,并发症发生率较常规组明显降低,差异有统计学意义(P<0.05)。末次随访时常规组与3D打印组疼痛VAS评分与髋关节功能Harris评分比较差异无统计学意义(P>0.05)。结论3D打印技术在全髋关节置换术治疗复杂髋部疾病时具有重要的临床应用价值,术者可在术前充分了解髋部局部解剖情况并制定合理的手术方案,并且可以缩短手术时间、减少术中出血量、降低术后并发症发生率。

Abstract:Objective To evaluate the clinical value of 3D printing technology in total hip arthroplasty for complex hip diseases.Methods Thirty-seven patients(42 hips)with complex hip diseases were treated by total hip arthroplasty from June 2016 to December 2018.All patients were divided into two groups,19 patients(22 hips)underwent routine operation(routine group)and 18 patients(20 hips)underwent 3D printing technology assisted surgery(3D printing group).The operation time,intraoperative blood loss,complication rate,and VAS score and Harris score of hip joint function at the last follow-up were compared between the two groups.Results Thirty-seven patients were followed up for an average of 23.5(6-48)months.The operation time of 3D printing group was shorter than that of routine group,the bleeding amount and the complication rate were lower than that of routine group(P<0.05).At the end of follow-up,there was no significant difference in VAS score and Harris of hip function between two group(P>0.05).Conclusion Application of 3D printing technology is of great clinical value in the treatment of complex hip diseases with total hip replacement.The surgeon can fully understand the local anatomy of the hip before operation and make a reasonable operation plan.It can shorten the operation time,reduce the amount of intraoperative bleeding and reduce the postoperative complication rate.
