乳腺肿块能量多普勒超声与微血管密度的相关性研究

摘    要:【目的】探讨能量多普勒超声(power Doppler imaging,PDI)与微血管密度(microvessel density,MVD)检测乳腺肿块血管生成的相关性并评价其临床应用价值。【方法】超声检测乳腺肿块患者55例(其中良性组25例,恶性组30例),采用计算机对肿块能量多普勒血流信号进行定量分析,测定彩色像素密度(color pixel density,CPD);抗CD34因子免疫组化染色测定肿块MVD,对CPD与MVD的相关性进行分析。【结果】恶性组CPD、MVD较良性组高,两者差异有显著性意义(P〈0.01)。恶性组中腋下淋巴结转移组的CPD及MVD均高于无腋下淋巴结转移组(P〈0.01)。良恶性肿块的CPD与MVD间具有良好相关性(r=0.846,r=0.889,P〈0.01)。【结论】能量多普勒超声所测CPD可较好反应乳腺肿块血流情况,有助于乳腺疾病的鉴别诊断,与微血管密度计数相结合,能全面反映肿瘤血管生成状态。

Abstract: Objective] To study the correlation between power Doppler imaging and microvessel density (MVD) about breast masses angiogensis and to valuate the clinical application. Methods] Power Doppler ultrasound was performed on 55 patients with breast mass (25 benign, 30 malignant) .Color pixel density (CPD) with computer-assistant of power Doppler flow signal were analyzed. MVD was calculated in surgical specimen after immunohistochemical staining using anti- factor CD34. The correlation between CPD and MVD was analyzed. Results] The CPD and MVD in malignant group was higher than that in benign group ( P 〈 0. 01 ) . Axillary node - positive tumors demonstrated significantly higher CPD and MVD than did axillary node - negative tumors (P〈0.01), and good correlation was shown between CPD and MVD (r=0.846, r=0.889, P〈0.01) . Conclusions] Power Doppler CPD as a indicator for breast mass angiogenesis is useful for differential diagnosis of breast diseases. Power Doppler imaging quantafive assessement integrated with MVD can observe intratumoral vaseularizafion overall.
