摘    要:目的探究Monitored Rehab Systems下肢智能运动训练系统对脑卒中偏瘫患者平衡功能及步行功能的作用及对患者远期预后的影响。方法选取105例脑卒中偏瘫患者为研究对象,采用简单随机化分组方法分为对照组52例和观察组53例。对照组采用常规康复训练方法,观察组在常规训练基础上采用Monitored Rehab Systems下肢智能运动训练系统。对比两组脑卒中偏瘫患者治疗前和治疗4、8、12周后FMA、BBS、BI评分、FAC评分,同时对比两组脑卒中偏瘫患者远期预后情况。结果两组治疗4、8、12周后FMA、BBS评分均高于治疗前(P<0.05),治疗8、12周后FMA、BBS评分高于治疗4周后(P<0.05),治疗12周后FMA、BBS评分高于治疗8周后(P<0.05),且观察组治疗后不同时间点FMA、BBS评分均高于对照组(P<0.05);两组治疗4、8、12周后FAC评分均高于治疗前(P<0.05),治疗8、12周后FAC评分高于治疗4周后(P<0.05),治疗12周后FAC评分高于治疗8周后(P<0.05),同时观察组治疗后不同时间点FAC评分均高于对照组(P<0.05);两组患者治疗前、治疗后和治疗后6个月NIHSS评分和Barthel指数情况对比均差异统计学意义(P<0.05),两组治疗后、治疗后6个月NIHSS评分均低于治疗前(P<0.05),同时Barthel指数均高于治疗前(P<0.05),治疗后6个月NIHSS评分和Barthel指数分别低于和高于治疗后(P<0.05),且观察组治疗后不同时间点NIHSS评分均低于对照组(P<0.05),同时Barthel指数均高于对照组(P<0.05)。结论Monitored Rehab Systems下肢智能运动训练系统能有效改善脑卒中偏瘫患者的平衡功能及步行功能,且能改善远期预后。

Abstract:Objective To investigate the effect of the lower limb intelligent exercise training system Monitored Rehab on balance function, walking function and long-term prognosis of stroke patients with hemiplegia. Methods A total of 105 stroke patients with hemiplegia admitted to Wenzhou Hospital of traditional Chinese medicine and the Second Affiliated Hospital of Wenzhou Medical University from January to September 2019 were enrolled in the study. A simple randomized grouping method was applied to assigned 52 patients in the control group and 53 patients in the observation group. The control group used the conventional rehabilitation training method, and the observation group used the Monitored Rehab lower limb intelligent exercise training system on the basis of routine training. The FMA, BBS, BI score, MI-L, MAS, FAC score, step size, pace, and stride frequency were compared between the two groups before and after 4, 8 and 12 weeks of treatment. Long-term prognosis of patients with hemiplegia was also compared. Results FMA and BBS scores were higher in the two groups after 4, 8 and 12 weeks treatment (P<0.05). After 8 and 12 weeks treatment, the FMA and BBS scores were significantly higher than those after 4 weeks treatment (P<0.05). The scores of FMA and BBS after 12 weeks of treatment were significantly higher than those after 8 weeks of treatment (P<0.05); and the scores of FMA and BBS in the observation group were significantly higher than those in the control group at different time points after treatment (P<0.05). After 4, 8 and 12 weeks treatment in the two groups, FAC scores were higher than before treatment (P<0.05). After 8 and 12 weeks of treatment, FAC scores were significantly higher than 4 weeks after treatment (P<0.05). FAC score after 12 weeks of treatment was significantly higher than those after 8 weeks of treatment (P<0.05). FAC scores were significantly higher in the observation group than in the control group at different time points (P<0.05). The NIHSS scores in the 2 groups after treatment and 6 months after treatment were significantly lower than those before treatment (P<0.05), and the Barthel indexes were significantly higher than before treatment (P<0.05). The NIHSS scores of the observation group were significantly lower than the control group at different time points after treatment (P<0.05); while the Barthel indexes were significantly higher than the control group (P<0.05).
