慢性肺源性心脏病中医常见证候临床调查研究

目的基于国内4家中医院呼吸科719例慢性肺源性心脏病临床调查数据分析,探讨中医证候分布规律及其特征。方法结合前期文献调查研究结果设计临床调查表,对四家中医院呼吸科近三年收治患者进行临床调查,采用SPSS13.0统计软件运用Logistic回归、因子分析和聚类分析等方法对证候分析。结果痰热壅肺、痰湿阻肺、痰淤阻肺、阳虚水泛、肺肾气虚、肺肾阴虚、肺肾气阴两虚等以及血瘀兼证是其常见证候。痰热壅肺、痰湿阻肺、肺肾阴虚、肺肾气阴两虚、阳虚水泛、血淤证、心肺气虚的证候属性比较稳定。结论慢性肺源性心脏病的常见证候有痰热壅肺、心肺气虚等八证以及血淤兼证,证候属性有一定规律性。

Clinical Investigation and Research of TCM Common Syndrome for Chronic Pulmonary Heart Disease

Objective To explore the distribution regularity and characteristics of TCM syndromes for chronic pulmonary heart disease(CPHD).Methods The clinical questionnaire was designed in combination with previous literature research.The CPHD subjects were recruited from four TCM hospital respiration departments.The research data were analyzed with Logosit regression.Results The TCM common syndrome for CPHD included pattern of phlegm-heat obstructing lung,pattern of phlegm-damp stagnating lung,pattern of phlegm-blood stasis stagnating lung,pattern of edema due to "yang" deficiency,pattern of deficiency of pulmonary and renal "qi",pattern of deficiency of pulmonary and renal "yin",pattern of deficiency of pulmonary and renal "qi" and "yin" and blood stasis syndrome.The syndrome characteristics were stable of pattern of phlegm-heat obstructing lung,pattern of phlegm-damp stagnating lung,pattern of deficiency of pulmonary and renal "yin",pattern of deficiency of pulmonary and renal "qi" and "yin",pattern of edema due to yang deficiency,blood stasis syndrome and pattern of deficiency of pulmonary and heart "qi".Conclusion The common syndromes of CPHD include the pattern of phlegm-heat obstructing the lung,pattern of deficiency of pulmonary and heart qi and blood stasis syndrome etc.The syndrome characteristics are of regularity.
