脐血干细胞治疗缺血性心肌病的临床观察

目的观察脐血干细胞移植对缺血性心肌病患者治疗的效果及安全性。方法将我院心内科住院的缺血性心肌病患者64例，随机分为脐血干细胞治疗组和对照组。入院后所有患者均给予常规治疗。对照组经微导管于各冠状动脉远端注入10rIll肝素生理盐水；脐血干细胞治疗组经微导管于各冠状动脉远端注入10rnl脐血干细胞。观察治疗前后患者心功能、左心室舒张末期直径（LVEDd）、左室射血分数（LVEF）值，脑钠尿肽（BNP）水平和6分钟步行试验（6-MWT）的变化。结果治疗12周后，两组心功能分级、LVEF、BNP水平、6-MWT与治疗前比较均有明显改善（P〈0．01）；与对照组比较，脐血干细胞治疗组观察指标改善状况更加显著（P〈0．05）。结论常规治疗联用脐血干细胞治疗缺血性心肌病，患者可进一步获得有益的疗效，并且有良好的安全性和耐受性。

Clinical observation of stem cells on patients with ischemic cardiomyopathy

Objective Observation of umbilical cord blood stem cells in patients with isehemic cardiomyopathy treatment effects and safety. Methods 64 patients with ischemic cardiomyopatby in our hospital heart internal medicine resident were randomly divided into the treatment group of umbilical cord blood stem cells （treatment group） and control group. After admission the patients received conventional treatment that angiotensin converting enzyme inhibitors （ACEI）or angiotensin receptor antagonist, beta blockers ,digitalis, diuretics for conventional treatment, had reached the full dose of conventional treat- ment. Control group： the microcatheter in the distal coronary artery injection of 10ml heparin saline;cord blood stem cell group （ treatment group） ： the microdialysis catheters in the coronary artery distal to 10ml into the umbilical cord blood stem cells. Observation on heart function in patients before and after treatment, ultrasonic heartbeat of left ventricular end diastolic diameter （ LVEDd ） , left ventricular ejection fraction （ LVEF ）, BNP level and 6min walk test （ 6-MWT ） changes. Results Cord blood stem cell therapy for 12 weeks, two groups of heart function classification, LVEF, BNP, 6-MWT compared with before treatment had improved significantly （ P 〈 0. 01 ）, stem cell group compared with the control group, observation indicators had improved （ P 〈0. 05 ）. Conclusion Conventional treatment combined with umbilical cord blood stem cells in treatment of ischemic cardiomyopathy, patients can be further obtained good curative effect, and has good safety and tolerability.
