EB病毒抗体联合检测在鼻咽癌血清学诊断和筛查中的应用评价

摘    要:目的探讨单项和联合检测EB病毒(EBV)VCA-IgA、EA-D-IgG和EA-D-IgA 3种抗体在鼻咽癌血清学诊断及筛查中的价值。方法收集300例鼻咽癌患者血清,选择鼻咽良性疾病91例及可能与EB病毒感染有关的其他癌症患者100例为对照研究对象。用酶联免疫吸附试验(ELISA)对所有患者同时检测血清EBV VCA-IgA、EA-D-IgG和EA-D-IgA 3种抗体,评价单项和联合检测EB病毒抗体对鼻咽癌的血清学诊断和筛查的价值。结果单项EB抗体对鼻咽癌诊断的敏感性和特异性分别为:VCA-IgA 82.0%、85.7%,EA-D-IgG 84.6%、95.6%,EA-D-IgA 44.0%、97.8%,平行联合检测VCA-IgA和EA-D-IgG的敏感性为97.2%,大大高于各单项EBV抗体检测结果,差异有统计学意义(P<0.05),阴性预测值也高于VCA-IgA和EA-D-IgA,差异也有统计学意义(P<0.05)。结论联合检测EBVCA-IgA和EA-D-IgG有互补作用,二者平行联合检测可提高鼻咽癌血清学诊断及筛查的敏感性,序列联合检测可提高鼻咽癌血清学筛查的特异性。
