辅助导尿术在全身麻醉下行妇产科手术患者中的应用

目的 观察复方利多卡因乳膏辅助导尿术对全身麻醉下行妇产科手术后患者苏醒期躁动和术后尿道刺激症状的影响.方法 100例拟行妇产科手术患者均于全麻诱导插管后留置导尿管,随机分为两组.试验组50例在插管前用复方利多卡因乳膏润滑导尿管前端15 cm左右,按常规导尿术进行导尿；对照组50例采用传统的灭菌液状石蜡润滑导尿管进行导尿.评估两组患者留置导尿刺激症状严重程度.结果 实验组麻醉苏醒期躁动发生率(30.0％,15/50)明显低于对照组(86.0％ vs 43/50),差异有统计学意义(P＜0.05).实验组躁动的程度也轻于对照组,差异有统计学意义(P＜0.05).实验组术后2h和12h的尿道刺激症状明显轻于对照组,差异有统计学意义(P＜0.05),但术后24 h两组差异无统计学意义(P＞0.05).结论 复方利多卡因乳膏润滑导尿管可改善女性患者全身麻醉后苏醒期躁动,可减少术后留置尿管的尿道刺激症状的发生率及严重程度.

Effects of compound lidocaine cream on emergence agitation during the recovery period of general anaesthesia and urethral irritation due to indwelling urethral catheter

Objective To explore the effects of compound lidoeaine cream on emergence agitation during the recovery period of general anaesthesia and urethral irritation in patients undergoing indwelling urethral catheter.Methods One hundred patients were randomly assigned to receive either conventional catheterization using paraffin oil(the control group) or catheterization with application of compound lidocaine cream on the catheter(the study group).The emergence agitation during the recovery period of general anaesthesia and urethral irritation due to indwelling urethral catheter were compared between the two groups.Results There was statistically significant difference in emergence agitation during the recovery period of general anaesthesia between the two groups (30.0,15/50) vs(86.0%,43/50)],P<0.05.Statistcally significant difference was found in urethral pain at 2 and 12 hours after surgery between the two groups(P<0.05),while no statistically significant difference at was found at 24 hours(P>0.05).Conclusion Compound lidocaine cream is effective in relieving emergence agitation during the recovery period of general anaesthesia and reducing urethral irritation due to indwelling urethral catheter.
