219例肺癌就诊原因分析

目的：探讨肺癌患者就诊的原因及与肺癌分期的关系。方法：收集2000年北京协和医院住院治疗的肺癌患者219例的临床资料，其中鳞癌共55例，腺癌109例，小细胞肺癌21例，不能分型34例。结果：肺癌患者就诊原因多种多样，分为四类：体检发现，单纯咳嗽，胸部其他症状，胸外症状，其中体检发现为54例（24．7％），单纯咳嗽为67例（30．65），肺癌不同分期患者的就诊原因有显著性差异（P＜0．01），鳞癌中分期不同就诊原因无显著性差异（P＞0．05），而腺癌中分期不同就诊原因有性差异（P＜0．01）。结论：肺癌分期不同就诊原因明显不同，早期肺癌中大部分无任何症状或仅表现出咳嗽，提示了肺癌筛查在发现早期肺癌中十分重要。

An Analysis of Presenting Symptom in 219 Cases with Lung Cancer

To analyze presenting symptom of lung cancer and the relationsship between pre-senting symptom and TNM stage.Two hundred and nineteen patients with lung cancer admitted to Peking Union Hospital in2000were analyzed:55squamous cancer,109adenocarcinoma,21small cell lung cancer,and34undertermined.Many kind of presenting symptoms were reported,which were classi-fied into four groups:group1,detected by physical examination2,cough only,3,other pulmonary symptom,and4,extra-pulmonary symptom.Group1,54cases(24.7%)and group2,67cases(30.6%).It was found that pre-senting symptom was significantly different between the patients grouped according to TNM stages,and in adenocarcinoma.Conclusions]Majority of lung cancers are asymptomatic.It shows that screening plays im-portant role for early detection in lung cancer.
