Miles术后手指探入造瘘口小剂量甘油灌肠的临床应用

目的探讨直肠癌Miles术后造瘘口灌肠方法,比较灌肠效果。方法选择38例行直肠癌Miles术并在术后1周行扩肛、训练肠道功能、需要灌肠的患者,按手术单、双月进行分组,单月为普通组,双月为改进组,比较不同灌肠方法及灌肠过程中患者腹痛、舒适度及灌肠效果。结果改进组肛管插入均1次成功,灌肠过程中患者腹痛、不适感均低于普通组,灌肠效果明显提高。结论手指探入造瘘口小剂量甘油灌肠,可根据造口的方向、角度便于肛管插入,避免插管的盲目性,减少患者痛苦,同时还起到刺激肠蠕动、扩肛、预防造口狭窄并发症的作用。

Clinical application of small dosage of glycerin in the treatment of clyster by the way of placing fingers into stoma after Miles operation

Objective To explore the methods for stomal clyster after Miles operation and compare the clyster effects.Methods One week after Miles operation,38 patients in need of clyster were selected and divided into normal group(surgery in odd month) and modified group(surgery in even month).Clyster methods,level of abdominal pain,level of comfort and clyster effect were compared between two groups.Results Anal tubes were successfully inserted for only one time in modified group,and both of the levels of abdominal pain and discomfort in modified group were lower than the normal group.Clyster effect was improved significantly in modified group.Conclusion Clyster with small dosage of glycerin by placing fingers into stoma can make anal intubation easier according to the direction and angle of stoma,and it also can avoid blindness of placing anal tube and relieve the suffering of patients.Meanwhile,it plays the role of stimulating enterokinesia,expanding anus and preventing the complications of narrow stoma.
