卵巢无性细胞瘤PLAP、NSE和WT1检测的临床病理意义

摘    要:目的 :评价胎盘碱性磷酸酶 (PLAP) ,神经元特异性烯醇化酶 (NSE)、催乳素(PRL)和肾母细胞瘤 (WT1)基因蛋白在卵巢无性细胞瘤中的表达及其临床病理意义。方法 :复查 1983年 1月至 2 0 0 2年 10月曾经在重庆医科大学临床学院治疗的卵巢无性细胞瘤 31例的临床及病理资料。对 31例的肿瘤组织进行PLAP、NSE、PRL和WT1免疫组织化学染色。结果 :患者年龄 12~ 4 2岁 ,平均 2 5岁。临床Ⅰ期 18例 (5 8.1% ) ,Ⅱ期 5例 (16 .1% ) ,Ⅲ期 6例 (19.4 % ) ,Ⅳ期 2例 (6 .4 % )。PLAP、NSE、PRL和WT1在无性细胞瘤组织中的阳性表达率分别为 10 0 %、70 .9%、3.2 %和 12 .9%。NSE表达与临床分期以及 5年生存率之间显著相关 (P <0 .0 5 )。WT1的阳性表达与组织学亚型明显相关 (P <0 .0 5 )。结论 :NSE阳性表达与晚期肿瘤密切相关。PLAP和NSE可看作是无性细胞瘤重要的组织标志物 ,肿瘤组织是血清PLAP和NSE的主要来源。WT1的表达与无性细胞瘤分化较差有关

Abstract:Objective:To evaluate the expressions of placental alkaline phosphatase(PLAP),neuronspecific enolase(NSE),prolactin(PRL) and Wilm′s tumor(WT1) in ovarian dysgerminoma and their clinicopathological significance.Methods:Clinicopathological data were retrospectively reviewed in a total of 31 patients with pure dysgerminoma who were treated at College of Clinical Medicine,Chongqing Medical University from January 1983 to October 2002.Immunohistological staining for PLAP,NSE,PRL and WT1 was performed in all 31 tumor tissues.Results:The age of the patients ranged from 12 to 42 years (average 25 yrs).According to the clinical staging,stage I:18 (58.1%),stage II:5(16.1%),stage III:6(19.4%),and stage IV:2(6.4%).Of the 31 cases,the positive expression rates of PLAP,NSE,PRL and WT1 were 100%(31/31),70.9%(22/31),3.2%(1/31) and 12.9(4/31) respectively.There was a significant relationship between NSE expression and clinical stages (P<0.05) and 5 year survival rate (P<0.05).The positive expression rate of WT1 was significantly related to histological types (P<0.05).Conclusions:NSE positive expression is closely related with advanced tumor stage (stage Ⅲ/Ⅳ).PLAP and NSE can be considered as important markers of dysgerminoma tissues,and the tumor tissues are the main resources of serum NSE and PLAP.WT1 expression correlated with poorer differentiaton of dysgerminoma.
