锁孔手术三种骨孔修复技术的对比观察(附64例分析)

摘    要:目的对比锁孔入路显微手术3种骨孔修复技术的优势及并发症。方法回顾性分析64例行锁孔手术的病人资料,分别采用3种骨孔修复技术,其中骨屑回植法34例,丝线固定法15例和颅骨锁固定法15例。结果骨屑回植法开关颅时间较短,手术费用低,但皮下积液的发生率较高(20.6%)。丝线固定法操作简单,节省费用,但发生骨瓣漂浮2例。颅骨锁固定法可消除锁孔手术的"关键孔",不仅固定效果好,并发症少,而且可使其真正"无孔化"。结论颅骨锁固定法在锁孔手术的骨孔修复上,较骨屑回植法和丝线固定法更具有优势。

Abstract:Objective To compare the advantages and complications of three kinds of repair technique for skull defect in minimally invasive microsurgery via keyhole approach.Methods The clinical data of 64 patients were analyzed retrospectively.Three kinds of bone repair technique were used, including autologous bone chip replantation in 34 cases, silk suture fixation in 15 and skull lock fixation(titanium screw fixation of the skull) in 15.Results Autologous bone chip replantation has relatively low expenditure with short operative time, but it has a high incidence of subcutaneous effusion(20.6%).Silk suture fixation technique was convenient and economic, but floating bone flap was observed in 2 cases.Skull lock fixation technique eliminated the "key hole" with efficient fixation and fewer complications.Conclusion Skull lock fixation is superior to autologous bone chip replantation and silk suture fixation for bone defect repair via keyhole approach.
