米索前列醇在晚期妊娠引产中的减量研究

目的 :探讨初产妇晚期妊娠引产时将米索前列醇的剂量降至 2 5μg能否有效 ,及能否降低副反应的发生。方法 :将妊娠 38～ 4 2周的健康初产妇 93例随机分为A组(2 5μg)和B组 (50 μg) ,将药物放入阴道后穹窿 ,每日 2次 ,间隔 6h ,最多应用 4次。 结果 :引产成功率A组为 80 % ,B组为 81.2 5% ,差异无显著性 (P >0 .0 5)。两组产妇的产程进展、分娩方式及新生儿结局均无显著性差异。但是 ,A组自用药到发动规律宫缩的时间长于B组 ,产程中需用催产素加强宫缩的例数多于B组。副反应的发生率A组低于B组。结论 :将米索前列醇的剂量降至 2 5μg ,对晚期妊娠促宫颈成熟和引产有效 ,副反应发生减少 ,但是 ,孕妇对于米索前列醇的敏感性存在差异

Study of dose reduction of misopristol in induction of term pregnancy.

Objective:To study the effectiveness and side effects of misoprostol for cervical ripening and labour induction of term pregnancy when used in the dose of 25μg.Methods:93 healthy nulliparous women 38～42 weeks of gestation were randomized into group A(25μg)and group B (50μg) .They were received either 25μg or 50μg misoprostol vaginally two times a day,at the interval of 6 hours,to the maximum of 4 times.Results: There was a were no significant differences in the rate of success (group A 80% vs group B 81.25%, P>0.05).There were also no differences between the two groups in the duration of labour,the mode of delivery and the neonatal outcomes.The interval from the first dose to the initiation of labour in group A was longer than that of group B.More women in group A need oxytocin administration to enhance the uterine contraction than that in group B. Conclusions:Misoprostol 25μg is as effective as 50μg used vaginally in cervical ripening and induction of labour in term pregnancy.Less side effects are found at the dosage of 25μg.But there are some differences in women at term pregnancy in the sensitivity to misoprostol.
