超声造影在子宫肌瘤与子宫腺肌病鉴别诊断中的价值

摘    要:目的探讨超声造影在子宫肌瘤与子宫腺肌病鉴别诊断中的价值。方法对56例子宫肌瘤及23例子宫腺肌病患者进行超声造影检查。结果子宫肌瘤表现为假包膜内血管首先灌注呈环状高增强,并分支进入瘤体内部,随后整个瘤体表现为均匀或不均匀高增强,变性坏死区无增强。造影后期,瘤体内部造影剂消退较正常肌层快呈低增强,而假包膜消退相对较慢始终呈稍高增强,有明显包膜感。子宫腺肌病表现为多条血管呈不规则分支状进入病灶内,整个病灶区肌层呈不均匀高增强。造影后期呈不均匀稍低增强,边界不清,无包膜感。结论子宫肌瘤与子宫腺肌病超声造影表现明显不同,将为两者的鉴别诊断提供更加准确的信息。

Abstract:Objective To explore the value of contrast-enhanced ultrasound(CEUS) in the differential diagnosis of hysteromyoma(HM)and adenomyosis(AM).Methods Fifty-six cases with HM and 23 cases with AM were studied with CEUS.Results After contrast administration,HM was demonstrated peripheral pseudocapsule enhancement with branched vessels from exterior to interior,and then the entire masses were enhanced homogeneously or heterogeneously with non-enhancement of degenerative or necrotic areas.HM exhibited earlier washout than that of myometrium in the late phase,while the pseudocapsule still remained hyper-enhancement with obvious membraniform sign.AM depicted several irregular branched feeding vessels,and the whole infiltrated muscular tissue was enhanced heterogeneously,and then subsided to hypo-enhancement in the late phase with unclear margin and without membraniform sign.Conclusions HM and AM showed different enhancement patterns,which may provide us some exact information in the differential diagnosis between the two diseases.
