喉癌功能保全性手术

本文对我国近十年来在喉癌外科治疗方面的进展进行了回顾。喉癌的外科治疗在根治肿瘤的前提下,喉功能的保留越来越受到重视。现代头颈外科技术的发展,要求术前和术中准确地判断肿瘤的部位和范围,术中根据肿瘤切除后喉部的缺损情况,采用不同方法进行修复和重建,以恢复喉的三大功能。实践证明只要正确掌握手术适应证,在相当一部分喉癌病例,喉部分切除术不但可以取得与喉全切除术相同的疗效,而且可保留喉的功能,提高患者的生存质量。

The surgical treatment of laryngeal carcinoma has improved greatly in the last decade. Not only the radical removal of tumor, but also the reservation of the laryngeal functions has been more and more emphasized, The development of modern laryngology requires precise evaluation of the site and the extension of the tumor preoperatively, in order to employ different techniques of partial laryngectomy for different tumors. Clinical evidence has shown that if the surgical indication is posed appropriately, partial laryngectomy not only can obtain the same long term survival results as that of total laryngectomy, but also can reserve laryngeal functions and improve the patientl' s quality of life.
