高频彩色多普勒超声在甲状腺癌诊断中的应用价值

目的:观察各类甲状腺癌的超声图像特征,探讨高频彩色多普勒超声技术对甲状腺癌的诊断价值。方法:对术后病理证实的20例甲状腺癌患者的超声图像特征及病理分型进行回顾分析。结果:20例甲状腺癌病例中乳头状癌13例,滤泡状腺癌4例,未分化癌2例,髓样癌1例,各类甲状腺癌超声像图及临床表现具有特征性,如乳头状癌:此类癌多见,恶性程度低,微钙化是其特征;滤泡状癌:呈较高回声,一般有包膜,穿入型滋养动脉是其血供特征,常见于50岁左右中年人;未分化癌:无包膜,常有坏死出血,高度恶性,多见于70岁以上老年人;峡部的肿瘤结节恶性率较高。结论:甲状腺癌超声图像具有特征性表现,并与病理类型有关,高频彩色多普勒超声诊断与病理诊断符合率高,且操作方便,是临床上影像检查甲状腺癌的首选和主要方法。

High-frequency color Doppler ultrasound in the diagnosis of thyroid cancer

Objective To observe the charactenrstics of the ultrasonic image sin ditferent thpes of thyroid carainoma and to disucss the diagnostic value of high:requcncy color Doppler ultrasound in thyroid carcinoma.Method Twenty cases finally proved with thyroid carcinoma were retrospectively analyzed of their ultrasonic image characteristics and pathological types.Results Among the 20 cases,papillary carcinoma were found in 13 cases,follicular adenocarcinoma in 4 cases,undifferentiated carcinoma in 2 cases and medullary carcinoma in 1 case.Different types of thyroid have different characteristics in ultrasonic images and clinical manifestation.Papillary carcinoma: common and with low malignant,characterized by micro-calcification.Follicular adenocarcinoma: common in the age of 50,showed with relatively hyperechoic,most have capsules,and characterized by penetrating vascular.Undifferentiated carcinoma: common in the age above 70,usually have no capsules,high malignant and might resulted in necrosis and hemorrhage.Conclusion High-frequency color Doppler is most suitable for scanning and detecting of thyroid carcinoma for its high coincidence rate with the pathological examination and its easy operation.
